Resumo

A doença de Chagas, descoberta por Carlos Chagas há aproximadamente 100 anos, ainda constitui um grave problema de Saúde Pública. É considerada doença extremamente negligenciada, uma vez que afeta exclusivamente as populações dos países em desenvolvimento. Os pacientes dessa doença são, em sua maioria, de baixa renda e, não representando mercado, ficam excluídos do escopo e dos esforços de Pesquisa e Desenvolvimento das Indústrias Farmacêuticas. Estima-se que entre 18 e 20 milhões de indivíduos estejam infectados pelo Trypanosoma cruzi, agente etiológico da doença e de, aproximadamente, 120 milhões, a população em risco de contaminação na América Latina. O tratamento da doença de Chagas continua sendo um desafio, pois apenas dois fármacos nitro-heterocíclicos são comercializados no mundo: nifurtimox (Lampit8, Bayer) e benznidazol (Rochagan8, Roche), sendo este último, o único fármaco disponível no mercado brasileiro. Dentre os compostos potencialmente antichagásicos em estudo, o hidroximetilnitrofural (NFOH) mostrou maior atividade tripanomicida e menor mutagenicidade do que o nitrofural (composto matriz) em todos estágios de desenvolvimento de um fármaco. Em 2006, os estudos pré-clínicos de toxicidade mostraram que o NFOH apresentou DL50 >2000 mg em ratos, dose considerada segura pela Organização Mundial da Saúde (OMS) e promissor para a continuidade dos ensaios. Nesse sentido, o objetivo deste trabalho foi estudar a conversão in vitro e in vivo do NFOH em NF. Assim, metodologia analítica foi desenvolvida e validada para a determinação de NF e NFOH em matriz de plasma humano e rato e foram realizados estudos de conversão in vitro (pH 1,2 e 7,4) e ensaios in vivo (farmacocinéticos) em ratos. Chagas disease, discovered by Carlos Chagas about a century ago and also known as American trypanosomiasis, is still a serious Public Health problem. The disease is considered extremely neglected because it afflicts exclusively populations in developing countries. Most Chagas’ disease patients fall within the low income bracket and, being economically insignificant, are excluded from the research and development scope and efforts of the Pharmaceutical Industry. It is estimated that 18 and 20 million people in Latin America are infected with Trypanosoma cruzi, the etiologic agent of the disease, and that approximately 120 million people are at risk of contamination in the continent. Chagas’ disease treatment is a challenge because only two nitroheterocyclic drugs are market worldwide, namely nifurtimox (Lampit8, Bayer) and benznidazol (Rochagan8, Roche), and the latter is the only drug available in the Brazilian market. Among the potentially anti-Chagas compounds under study, hydroxymethyl nitrofural (NFOH) has shown higher tripanomicide activity and lower mutagenicity than nitrofural (active compound) in all stages of development of the parasite. In 2006, preclinical toxicity studies showed that NFOH presented LD50 higher than 2000 mg in rats. This value is considered safe by the World Health Organization (WHO) and promising for continuing test. In this context, the purpose of this work was to investigate the in vivo stability of NFOH. To this end, an analytical methodology was designed and validated for the determination of NF and NFOH in human and rat blood plasma. Conversion studies were carried out in vitro (pH 1.2 and 7.4) and in vivo (pharmacokinetic studies) with rats.
