Efeitos da eletroestimulação transcutânea do nervo tibial sobre a variabilidade da frequência cardíaca nos índices lineares e não lineares

Resumo

Introdução: A incontinência urinária (IU) é um sintoma que vai se caracterizar pela perda involuntária de urina, causando problemas higiênicos, sociais, psicológicos. Existem três tipos de IU e uma delas é a de urgência (IUU). A eletroestimulação transcutânea do nervo tibial (ETNT) pelo aparelho TENS é um dos tratamentos mais eficazes para esse tipo IU. A população mais atingida pela IUU são as mulheres que estão na menopausa e neste período é comum elas sofrerem com patologias cardíacas, por este motivo, torna-se importantíssimo identificar o que podem ser fatores de risco cardiovasculares. Um dos métodos que podemos utilizar para avaliar esse estresse cardíaco é a Variabilidade da Frequência Cardíaca (VFC). Objetivo: Verificar se a aplicação do TENS pode alterar a modulação autonômica da frequência cardíaca e se este tratamento é seguro. Métodos: 15 mulheres na menopausa e com IUU foram submetidas a um protocolo experimental com três etapas: Análise da VFC durante dez minutos de repouso, ETNT por vinte minutos e Análise da VFC durante período de trinta minutos de recuperação. Os parâmetros frequência cardíaca (FC), pressão arterial (PA), frequência respiratória(f) e os índices da VFC, no domínio do tempo (RMSSD, PNN50, SDNN), da frequência (LF e HF(ms2) e LF/HF) e geométrico (SD1), foram observados durante o repouso e na recuperação. Para as comparações dos valores dos índices da VFC entre os momentos (repouso vs. períodos de recuperação) foi utilizado o teste de ANOVA uma via seguida pelo pós-teste de Bonferroni para distribuições paramétricas ou o pós-teste de Newman Keuls para distribuições não paramétricas. A significância estatística foi fixada em 5% para todas as análises. Resultados: Não foram encontradas diferenças entre os momentos entre repouso e recuperação pós ETNT. Conclusão: A ETNT não foi capaz de influenciar a regulação autonômica dessas mulheres.

One of the methods we can use to evaluate cardiac stress this is the Heart Rate Variability (HRV). Objective: To determine whether the application of TENS can change the autonomic modulation of heart rate and if this treatment is safe. Methods: 15 postmenopausal women with UUI and were subjected to an experimental protocol with three steps: HRV analysis for ten minutes of rest, TETN for twenty minutes and Analysis of HRV during thirty minutes of recovery. The parameters heart rate (HR), blood pressure (BP), respiratory rate (f) and the indices of HRV in the time domain (RMSSD, PNN50, SDNN), frequency (LF and HF (ms2) and LF / HF ) and geometric (SD1), were observed during rest and recovery. For comparisons of the values ​​of HRV indices between times (vs. rest periods of recovery) was used ANOVA one way followed by Bonferroni post-test for parametric distributions or the Newman Keuls post-test for nonparametric distributions . Statistical significance was set at 5% for all analyzes. Results: No differences were found between the moments of rest and post TETN recovery. Conclusion: TETN was unable to influence the autonomic regulation of these women.
