Resumo
Após a extração dentária, a pneumatização do seio maxilar associada ao processo de remodelação alveolar favorece a perda significativa em altura do rebordo alveolar, e quando esta atrofia atinge valores iguais ou menores que 5mm, a opção de escolha da técnica é realizar o levantamento do assoalho do seio maxilar associado ao uso de biomaterial. No entanto, as complicações intra e pós operatórias são diversas, além da necessidade da espera de um período de até 6 meses para a inserção do implante e 6 meses para a fase protética. Portanto o objetivo deste estudo foi avaliar a formação óssea peri-implantar a partir do rebordo alveolar remanescente em altura com o uso de implante cone morse imediatamente após o levantamento do seio maxilar sem o uso de biomaterial e instalação imediata da prótese. Foi realizada a instalação de 11 implantes (Grand Morse®, Neodent, Curitiba-PR) de 13mm de comprimento por 4,3mm de diâmetro logo após o levantamento da membrana sinusal localizada no seio maxilar em pacientes com osso remanescente de 5 mm somente. A confecção e instalação da prótese provisória foi realizada dois dias após o procedimento cirúrgico. A prótese definitiva foi instalada aos 6 meses após a fase cirúrgica. Após o acompanhamento de 12 meses, foi realizada a avaliação do índice de sobrevivência dos implantes nas tomografias com a finalidade de observar se houve formação óssea a partir do coágulo no rebordo alveolar remanescente. Como resultado, houve um aumento da altura óssea no período de 12 meses, com resultados estatisticamente significativos quando comparado ao período inicial, assim como quando comparado 6 e 12 meses; e unicamente no nível palatino quando comparado o período inicial e 6 meses. Dessa forma, houve aumento de altura óssea peri-implantar a partir do osso remanescente em altura, com a técnica de levantamento de assoalho do seio maxilar sem biomaterial e prótese imediata, com implantes de comprimento regular e hidrofóbicos.
After tooth extraction, the pneumatization of the maxillary sinus associated with the alveolar remodeling process favors a significant loss in height of the alveolar ridge, and when this atrophy reaches values equal to or less than 5mm, the choice of technique is to perform a maxillary sinus floor lift associated with the use of biomaterial. However, there are a number of intra- and post-operative complications, as well as the need to wait up to 6 months for the implant to be inserted and 6 months for the prosthetic phase. Therefore, the aim of this study was to evaluate peri-implant bone formation from the remaining alveolar ridge height using a Morse cone implant immediately after maxillary sinus lift without the use of biomaterial and immediate installation of the prosthesis. Eleven implants (Grand Morse®, Neodent, Curitiba-PR) measuring 13mm in length and 4.3mm in diameter were installed immediately after lifting the sinus membrane located in the maxillary sinus in patients with only 5mm of bone remaining. The provisional prosthesis was made and installed two days after the surgical procedure. The definitive prosthesis was installed 6 months after the surgery. After the 12-month follow-up, the survival rate of the implants was assessed on CT scans in order to see if there was bone formation from the clot in the remaining alveolar ridge. As a result, there was an increase in bone height at 12 months, with statistically significant results when compared to the initial period, as well as when comparing 6 and 12 months; and only at the palatal level when comparing the initial period and 6 months. Thus, there was an increase in peri-implant bone height from the remaining bone in height, with the maxillary sinus floor lift technique without biomaterial and immediate prosthesis, with regular length and hydrophobic implants.
