A radioterapia está associada a diversos efeitos adversos, sendo a mucosite oral radioinduzida uma das complicações mais frequentes. Este estudo avaliou o grau de mucosite oral radioinduzida, dor, níveis de cortisol salivar e ansiedade e depresão em pacientes com câncer de cabeça e pescoço sob tratamento radioterápico. Os pacientes foram acompanhados antes do ínicio do tratamento (T0), ao longo de várias sessões de radioterapia (T5, T15, T25, T30) e após 30 dias da conclusão da radioterapia (PT). A mucosite foi avaliada pela classificação da Organização Mundial da Saúde (OMS) e de acordo com a região anatômica afetada. A intensidade da dor foi avaliada pela Escala Visual Analógica (EVA). Os sintomas de ansiedade e depressão foram avaliados a partir do questionário Escala Hospitalar de Ansiedade e Depressão. Os níveis de cortisol salivar foram mensurados pelo kit de Ensaio Imunoenzimátio (ELISA). Os resultados demonstraram que a gravidade da mucosite bucal aumentou significantemente durante o tratamento em T15 (P =.0005),T25 (P =.0058) e T30 (P =.0013), bem como as áreas afetadas (P =.0003; P =.0017; P =.0039). A dor relacionada a mucosite bucal também apresentou aumentos significativos em T15 e T25 (P =.0196 e P =.0116). Os sintomas de ansiedade e depressão não demonstraram diferenças significativas. Já os níveis de cortisol salivar aumentaram significativamente em T15, T25 e T30 (P =.0003; P <.0001; P <.0001). Concluiu-se que, durante o tratamento oncológico, foi observado um aumento significativo na gravidade da mucosite bucal induzida por radioterapia e em dor associada as lesões e dos níveis de cortisol.

Abstract (english)

Radiotherapy is associated with various adverse effects, with radiation-induced oral mucositis being one of the most common complications. This study assessed the degree of radiation-induced oral mucositis, pain, salivary cortisol levels, and anxiety and depression in head and neck cancer patients undergoing radiotherapy treatment. Patients were monitored before the start of treatment (T0), during several sessions of radiotherapy (T5, T15, T25, T30), and 30 days after the completion of radiotherapy (PT). Mucositis was evaluated using the World Health Organization (WHO) grading system and assessed according to the affected anatomical region, in addition to pain, which was evaluated using the Visual Analog Scale (VAS). Anxiety and depression symptoms were assessed using the Hospital Anxiety and Depression Scale (HADS). Salivary cortisol levels were measured using an Enzyme-Linked Immunosorbent Assay (ELISA) kit. The results showed that the severity of oral mucositis significantly increased during treatment at T15 (P = 0.0005), T25 (P = 0.0058), and T30 (P = 0.0013), as well as the affected areas (P = 0.0003; P = 0.0017; P = 0.0039). Pain related to oral mucositis also showed significant increases at T15 and T25 (P = 0.0196 and P = 0.0116). Anxiety and depression symptoms did not show significant differences. Salivary cortisol levels significantly increased at T15, T25, and T30 (P = 0.0003; P < 0.0001; P < 0.0001). It was concluded that, during oncological treatment, a significant increase was observed in the severity of radiation-induced oral mucositis and pain associated with the lesions and salivary cortisol levels.
