Sistemas precursores de cristais líquidos mucoadesivos para administração bucal de curcumina associados à terapia fotodinâmica no tratamento da candidose bucal.

Resumo

A candidose bucal é causada principalmente pela levedura oportunista Candida albicans. O tratamento envolve a administração de antifúngicos de uso tópico ou sistêmico, para os quais se tem observado casos de resistência. Pesquisadores estão buscando métodos alternativos, como a terapia fotodinâmica antimicrobinana e a descoberta de substâncias de origem natural que possam apresentar interesse terapêutico no tratamento da candidose bucal, como a curcumina, cuja ação antifúngica pode ser potencializada pelo emprego da luz, uma vez que atua também como fotossensibilizador. Todavia, esse fármaco apresenta características físico-químicas limitantes, como baixa solubilidade aquosa. Logo, sua incorporação em sistemas precursores de cristais líquidos (SPCL) acrescidos de polímeros mucoadesivos demonstra ser uma opção interessante para viabilizar o seu uso. O trabalho objetiva a avaliação de SPCL mucoadesivos para administração bucal de curcumina, bem como verificar o efeito do tratamento da candidose bucal com o uso da terapia fotodinâmica, empregando estudos in vitro e in vivo. Para isso, foram desenvolvidos SPCL compostos de ácido oleico, álcool cetílico etoxilado e propoxilado (Procetyl® AWS) e dispersão polimérica de poloxamer (16%) e/ou polietilenoimina (0,5%), capazes de incorporar a curcumina, os quais foram caracterizados empregando microscopia de luz polarizada (MLP), reologia, espalhamento de raios X de baixo ângulo (SAXS) e mucoadesão in vitro, antes e após a diluição com saliva artificial. Foram realizados estudos de liberação in vitro, permeação e retenção ex vivo, além de ensaios biológicos in vitro e in vivo para a avaliação da curcumina incorporada em SPCL frente a Candida albicans. Na MLP, foi possível a visualização da transição dos SPCL, caracterizado como campo escuro, para mesofases hexagonais e lamelares, características de sistemas líquido-cristalinos. Os resultados reológicos evidenciaram que os sistemas obtidos apresentaram-se inicialmente como Newtonianos e após a adição da saliva artificial mostraram-se como não Newtonianos, pseudoplásticos. Os resultados de SAXS corroboraram com os resultados de MLP. No ensaio de mucoadesão in vitro foi observado que houve maior força de atração entre a mucosa e a formulação nas amostras que continham a união dos dois polímeros e após a adição da saliva artificial. No ensaio de liberação in vitro foi observado que os sistemas modificaram o perfil de liberação da curcumina quando comparado com o fármaco em solução. No ensaio de permeação ex vivo foi observado que os sistemas não foram capazes de permearem a membrana esofágica; contudo, quantidade significativa de curcumina foi detectada no ensaio de retenção. No ensaio de viabilidade celular evidenciaram que após 24h do SPCL em contato com as células testadas, estas ainda apresentaram 75% de células viáveis quando comparado ao controle de células vivas. O ensaio in vivo mostrou que após 24h de tratamento não houve efeito antimicrobiano em relação ao controle positivo. Contudo, os dados obtidos dos animais analisados após 7 dias do último tratamento, mostram promitentes, uma vez que em alguns dos animais não tiveram crescimento de colônias no grupo PP-CUR-PDT. Na análise histológica dos animais analisados após 24h do último tratamento, foi observada a presença de pseudo-hifas de Candida albicans no tecido epitelial dos animais tratados apenas PP-CUR sem irradiação de luz; todavia, os animais tratados com a formulação de curcumina em associação a luz não apresentaram a levedura segundo análise histológica. Logo, os resultados obtidos neste trabalho sugerem que os sistemas desenvolvidos e caracterizados apresentaram características almejadas no tratamento da candidose bucal, de modo a facilitar a aplicação e permanência na mucosa da boca, sendo também capaz de controlar a liberação da curcumina.
Oral candidiasis is caused mainly by the opportunistic yeast Candida albicans. Treatment involves the administration of topical or systemic antifungal for which resistance has been observed. Researchers are looking for alternative methods, such as photodynamic therapy and the discovery of substances of natural origin that may be of therapeutic interest in the treatment of oral candidiasis, such as curcumin, whose antifungal action may be enhanced by the use of light, it also acts as a photosensitizer. However, this drug has limiting physical-chemical characteristics, such as low aqueous solubility. Therefore, its incorporation in liquid crystal precursor systems (LCPS) plus mucoadhesive polymers proves to be an interesting option to make its use feasible. The objective of this study is to evaluate the LCPS mucoadhesive for buccal administration of curcumin, as well as to verify the effect of oral candidosis treatment with the use of photodynamic therapy, using in vitro and in vivo studies. For this, LCPS compounds of oleic acid, ethoxylated and propoxylated cetyl alcohol (Procetyl® AWS) and polymer dispersion of poloxamer (16%) and/ or polyethyleneimine (0.5%), capable of incorporating curcumin, were developed. The compounds were characterized by using polarized light microscopy (PLM), rheology, scattering small angle X-ray scattering (SAXS) and mucoadhesion in vitro, before and after dilution with artificial saliva. In vitro release, permeation and retention ex vivo studies were carried out, as well as in vitro and in vivo biological assays for the evaluation of curcumin incorporated in LCPS versus Candida albicans. In PLM, it was possible to visualize the transition of LCPS, characterized as dark field, for hexagonal and lamellar mesophases, characteristics of liquid-crystalline systems. The rheological results evidenced that the systems obtained were initially Newtonian and after the addition of the artificial saliva were shown as non-Newtonian, pseudoplastic. SAXS results corroborated the results of PLM. In the in vitro mucoadhesion assay it was observed that there was greater force of attraction between the mucosa and the formulation in the samples that contained the union of the two polymers and after the addition of the artificial saliva. In the in vitro release assay it was observed that the systems modified the release profile of curcumin as compared to the drug in solution. In the ex vivo permeation assay it was observed that the systems were not able to permeate the esophageal membrane; however, significant amount of curcumin was detected in the retention assay. In the cell viability assay they showed that after 24 hours of the LCPS in contact with the tested cells, they still presented 75% of viable cells when compared to the control of living cells. The in vivo assay showed that after 24 hours of treatment there was no antimicrobial effect in relation to the positive control. However, data obtained in the animals analyzed after 7 days are promising, since complete killing was observed in some animals from the PP-CUR-PDT group. The histological images reveled presence of Candida albicans pseudo-hyphae in the epithelial tissue of the animals treated with PP-CUR in the absence of light irradiation. However, animals treated with the curcumin formulation in association with light did not present indicatives of yeast. Therefore, the results obtained in this work suggest that the developed and characterized systems presented characteristics desired in the treatment of oral candidosis, in order to facilitate the application and permanence in the mucosa of the mouth, being also able to control the release of curcumin.
