Resumo

Objetivo Geral: Elaborar um instrumento de registro da Sistematização da Assistência de Enfermagem (SAE) para a Sala de Recuperação Pós-anestésica (SRPA) com Diagnóstico de Enfermagem (DE), Intervenções de Enfermagem (IE) e Resultados de Enfermagem (RE). Objetivos específicos: Realizar o mapeamento cruzado entre a classificação de DE da NANDA-International (NANDA-I) e o conteúdo do registro de enfermagem utilizado na SRPA; Elencar os DE após o mapeamento cruzado; Propor para cada DE eleito as IE e respectivas atividades de enfermagem (AE) e RE e respectivos indicadores de resultados (IR) da Classificação das Intervenções de Enfermagem (NIC) e da Classificação dos Resultados de Enfermagem (NOC). Métodos: Trata-se de um estudo descritivo, exploratório e retrospectivo de análise documental, aprovado pelo Comitê de Ética e Pesquisa das instituições participantes, para a construção de um instrumento de registro manual do enfermeiro em uma SRPA de um hospital de referência em tratamento oncológico e realização de transplante de medula óssea no interior do Estado de São Paulo. O universo foi composto por 264 fichas pertencentes aos prontuários físicos com os dados: sinais vitais, necessidades afetas, prescrição médica e anotações de enfermagem. Os critérios de inclusão foram às fichas completas e preenchidas na SRPA entre 06h e 12h, de segunda-feira a sábado. A amostra constituiu-se por 187 fichas. A pesquisadora coletou os dados de junho a julho de 2018. A análise dos dados constituiu-se de três etapas: na primeira etapa, foram transcritos os dados das fichas analisadas que representavam os sinais e/ou sintomas significativos das necessidades e cuidados de enfermagem. Na segunda etapa foram eleitos os DE de risco e respectivos fatores de risco, e os DE com foco no problema e respectivas características definidoras e fatores relacionados, após o mapeamento cruzado entre os dados não padronizados coletados na primeira etapa e as classificações de DE da NANDA-I. Na terceira etapa, foram elaboradas listagens com o auxílio das classificações de enfermagem, NIC e NOC, e proposto IE e respectivas AE e RE e respectivos IR, para cada um dos DE identificados na segunda etapa, o que subsidiou o produto final. Resultados: Foram analisadas 187 fichas. Dos 13 Domínios da NANDA-I, cinco estão representados nesse estudo, sendo cinco DE de risco e 11 DE com foco no problema. Esses DE representam o perfil das necessidades de cuidados a partir do mapeamento cruzado entre o registro nas fichas e a classificação diagnóstica. Diante disso, foi construído um banco de dados com os DE, IE, AE, RE e IR. Conclusão: Os resultados do presente estudo contribuíram para a elaboração do produto final, como proposta de um instrumento de registro manual para a SAE na SRPA, priorizando a segurança e a qualidade prestada, com objetivo de prevenir as complicações e os riscos inerentes ao paciente, bem como, nortear a prática clínica e atribuir ao enfermeiro maior autonomia.
General Objective: To elaborate a registry of the Systematization of Nursing Assistance (SAE) for the Post-Anesthesia Recovery Room (SRPA) with Nursing Diagnosis (DE), Nursing Interventions (IE) and Nursing Results (RE). Specific Objectives: Carry out the cross-mapping between the NANDA-International classification of ED (NANDA-I) and the content of the nursing record used in the PACU; List EDs after cross-mapping; To propose for each ED elected IH and respective nursing activities (AE) and RE and respective outcome indicators (IR) of the Nursing Interventions Classification (NIC) and the Nursing Results Classification (NOC). Methods: This is a descriptive, exploratory and retrospective study of documentary analysis, approved by the Ethics and Research Committee of the participating institutions, for the construction of a manual registry of nurses in a PACU of a reference hospital in cancer treatment and bone marrow transplantation in the interior of the State of São Paulo. The universe consisted of 264 records belonging to the physical records with the data: vital signs, needs, medical prescription and nursing notes. The inclusion criteria were the complete files completed in the PACU between 06h to 12h from Monday to Saturday. The sample was 187 files. The researcher collected the data from June to July 2018. Data analysis consisted of three steps: The first step was to transcribe data from the files analyzed that represented the signs and/or significant symptoms of nursing needs and care. In the second stage, risk SDs and respective risk factors were selected, and EDs with a focus on the problem and their defining characteristics and related factors, after cross-mapping between the non-standard data collected in the first stage and the NANDA ED scores -I. In the third stage, lists were prepared with the help of the nursing classifications, NIC and NOC, and proposed IE and respective AE and RE and respective IR, for each of the ED identified in the second stage, which subsidized the final product. Results: 187 chips were analyzed. Of the 13 NANDA-I domains, five are represented in this study, with five SD at risk and SD 11 focusing on the problem. These EDs represent the profile of care needs from the cross-mapping between the record in the records and the diagnostic classification. Given this, a database was built with the DE, IE, AE, RE and IR. Conclusion: The results of the present study contributed to the elaboration of the final product, as a proposal of a manual registry instrument for SAE in the PACU, prioritizing the safety and quality provided, in order to prevent complications and risks inherent to the patient, as well as guiding clinical practice and assigning nurses greater autonomy
