Resumo

Introdução: O Acidente Vascular Cerebral é uma emergência médica com tempo ótimo para intervenção, afim de evitar sequelas e a morte do paciente. Devido ao tempo limítrofe, o acesso ao tratamento deve ser rápido e eficiente, e a responsabilidade da oferta de vaga é das centrais de regulações junto ao Núcleo Interno de Regulação (NIR). A atuação do enfermeiro nas centrais é inovadora e de extrema importância, porém pouco desenvolvida como pesquisa. Objetivos: realizar uma revisão integrativa sobre o tema; compreender a experiência dos enfermeiros do Núcleo Interno de Regulação dos casos de AVC em fase hiperaguda;e elaborar um vídeo de natureza instrucional para a capacitação de profissionais dos serviços de saúde. Métodos: A revisão integrativa foi elaborada por busca bibliográfica em bases de dados nacionais e internacionais com os descritores em equação booleana; a coleta dos dados qualitativos foi realizada por meio de entrevistas áudio-gravadas com 11 enfermeiros que possuíam vínculo empregatício no local do estudo escalados no Núcleo de regulação de vagas. O referencial metodológico utilizado para analisar os dados foi a Análise de Conteúdo de Bardin, vertente temática. O desenvolvimento do vídeo educativo utilizou a produção de um roteiro de imagens e etapas e a gravação de áudio. Para o desenvolvimento e consecução foram utilizados os seguintes softwares: Pixton; Adobe Photoshop; Camtasia; Powtoon. Os conteúdos inseridos no vídeo basearam-se nas 13 categorias e cinco temas que emergiram da análise de conteúdo das entrevistas. Resultados:Foram identificados nas bases nacionais e internacionais PUBMED; Scopus; Web os Science; Medline; EMBASE; Cinhal e LILACS 187 artigos utilizando uma equação Boleana com descritores Decs, cinco artigos foram identificados que responeram a pergunta norteadora “ Quais são as evidencias cientificas disponíveis na literatura relacionadas a protocolos de regulação de vagas no AVC em fase hiperaguda e qual a atuação do enfermeiro”, definiram-se 13 categorias e cinco temas após a análise das 11 entrevistas realizadas. As categorias que emergiram foram: Definição do termo AVC em janela; Tempo de Ictus; Incerteza quanto ao tempo de Janela; Responsabilidade e consciência profissional; Experiência com regulação de vagas; Protocolo Interinstitucional; Identificação dos sintomas; Processo de trabalho no serviço de regulação; Especificidades com as origens; Contato com as especialidades; Sistema de Informação; Qualificação na graduação; Motivação Positiva. Os temas delineados foram: Conhecimento sobre o AVC; Comprometimento do Profissional Enfermagem; Sistematização da Atividade Reguladora de vagas; Experiência Profissional do Enfermeiro; Relacionamento com os serviços externos. A maioria dos enfermeiros reconhece a importância de seu trabalho na Regulação da vaga e identifica falta de preparo dos serviços de origem do paciente ao preencher a ficha de encaminhamento ao serviço de Referência, o que resulta em atraso durante a regulação e perda de janela terapêutica ao paciente.Considerações Finais: Portanto, foi elaborado como produto um vídeo educativo com orientações de preenchimento da ficha para capacitação destes profissionais afim de otimizar o tempo de regulação com benefícios ao tratamento do AVC diminuindo sequelas e garantindo o acesso a assistência especializada e restabelecimento de saúde.
Introduction: Cerebrovascular accident is a medical emergency with optimal time for intervention in order to prevent sequelae and the death of the patient. Due to the time limit, access to treatment must be quick and efficient, and the responsibility for vacancy lies with the central regulators. The role of nurses in the centers is innovative and extremely important, but little developed as research. Objectives: To conduct an integrative review on the topic; to understand the experience of nurses of the Internal Nucleus of Regulation of cases of stroke in hyperacute phase, and to elaborate an instructional video for the qualification of health service professionals. Methods: the integrative review was elaborated by bibliographic search in national and international databases with the descriptors in boolean equation; Qualitative data were collected through audio-recorded interviews with 11 nurses who had employment at the study site assigned to the vacancy regulation center. The methodological framework used to analyze the data was the Bardin Content Analysis, thematic aspect. The development of educational video used the production of a script of images and steps and audio recording. For the development and achievement were used the following software: Pixton; Adobe Photoshop; Camtasia; Powtoon. The contents inserted in the video were based on the 13 categories and five themes that emerged from the content analysis of the interviews. Results: We use national and international PUBMED bases; Scopus; Web science; Medline; EMBASE; Cinhal and LILACS 187 articles using a Boolean equation with Decs descriptors, five articles were answered to a question to the guide “What is the scientific evidence available in the literature related to protocols for employment testing in hyperacute stroke and what is the role of the nurse ?13 categories and five themes were defined after analyzing the 11 interviews. The categories that emerged were: Definition of the term stroke in window; Ictus Time; Uncertainty about window time; Responsibility and professional awareness; Experience with job regulation; Interinstitutional Protocol; Identification of symptoms; Work process in the regulatory service; Specifics with origins; Contact with the specialties; Information system; Undergraduate qualification; Positive motivation. The themes outlined were: Knowledge about stroke; Commitment of the Nursing Professional; Systematization of the Regulatory Activity of vacancies; Nurse's Professional Experience; Relationship with external services. Most nurses recognize the importance of their work in the regulation of vacancy and identify lack of preparation of the patient's home services when filling out the referral form to the referral service, which results in delay during regulation and loss of therapeutic window to the patient. patient. Final considerations: Therefore, an educational video was prepared with guidelines to fill out the form to qualify these professionals in order to optimize the regulation time with benefits to stroke treatment, reducing sequelae and ensuring access to specialized care and health restoration.
