Introduction: Rapid Palatal Expansion (RPE) could increase nasal width, cross- sectional area and volume. However, the relationship between respiratory obstruction, malocclusion, and facial growth continues to be debated, due to studies finding methodological flaws such as no randomization and no control group. This research aimed to assess the dimensions of the pharyngeal airway space in growing patients after a RPE protocol. Methodology: A retrospective study was performed, through cephalometric evaluation in 30 patients. They were divided into 2 groups: Experimental group with 15 patients of both genders, aged 7-10 years in need of RPE, treated with Hyrax and maxillary constriction. Control group with 15 patients without ERP, paired in terms of gender and age, without transverse deficiency of the upper jaw. Airway measurements, Nasopharynx (NF), Oropharynx (OF) and Hypopharinx (HF) were performed on initial lateral head radiographs and 1 year after the ERP protocol ended. Results: ERP group presented an increase in the anteroposterior dimension in 2 regions, NF and OF, showing a statistical difference only in the first one (P <0.05) and a slight decrease in HF statistically non significant. In the control group, the 3 measured regions remained stable. Conclusion: Children with maxillary constriction the measurements of NF and OF may be decreased. ERP can significantly increase the measurement of NF in children with maxillary constriction during the period evaluated in this investigation.
