Efeitos da imunoterapia intravesical com Bacilo Calmette-Guerin (BCG) em associação ao treinamento físico aeróbio no tratamento do câncer de bexiga urinária não-músculo invasivo

Resumo

A terapia mais eficaz para o câncer de bexiga urinária não-músculo invasivo (CBNMI) é a imunoterapia com BCG (Bacilo Calmette-Guerin) associada à ressecção transuretral (RTU). Entretanto, a utilização do BCG está associada a vários efeitos colaterais de intensidades variadas, além da associação levar ao alto índice de recorrência tumoral. Além disso, não existem estudos mostrando o papel protetor do exercício físico aeróbio (EFA), como terapia adjuvante, sobre a evolução da carcinogênese urotelial. Assim, o presente estudo objetivou caracterizar e comparar as alterações morfológicas e moleculares da imunoterapia intravesical com BCG associada com o EFA na evolução do CBNMI induzido em ratos, enfocando os receptores do sistema imune inato, angiogênese e balanço oxidativo. Para a indução do CBNMI, vinte fêmeas da linhagem Fischer 344 receberam uma dose intravesical de 1,5 mg/Kg de N-metil-N-nitrosouréia (MNU) a cada 15 dias, totalizando 4 aplicações. Cinco fêmeas que não receberam MNU foram consideradas como Grupo Controle (Grupo 1) e receberam uma dose semanal intravesical de 0,3 mL de NaCl 0,9% por 6 semanas consecutivas. Duas semanas após a última aplicação da MNU, os animais foram divididos em quatro grupos (n=5 cada): Grupo MNU (Grupo 2): recebeu NaCl 0,9% por seis semanas consecutivas como o Grupo 1; Grupo MNU+BCG (Grupo 3): recebeu uma dose semanal intravesical de 2mg/mL de BCG por 6 semanas consecutivas; Grupo MNU+EFA (Grupo 4): recebeu o mesmo tratamento que os Grupos 1 e 2, e simultaneamente submetido ao protocolo de EFA (natação), ambos por 6 semanas consecutivas; e Grupo MNU+EFA+BCG (Grupo 5): recebeu uma dose semanal intravesical de BCG como descrito no Grupo 3 e simultaneamente submetido ao protocolo de EFA (natação), ambos por 6 semanas consecutivas. Após o tratamento, as bexigas urinárias foram coletadas e submetidas às análises histopatológicas e moleculares das proteínas TLR2,TLR4, NF-KB, IL-6, MMP-2, AKT, VEGF, Endostatina, NOX1, Nrf2, Bcl-2, BAX, B-actina . Os resultados demonstraram que os animais do Grupo 2 (MNU) apresentaram redução significativa no peso corporal e lesões uroteliais mais agressivas quando comparado aos demais grupos experimentais. No grupo com BCG e na associação entre BCG e EFA não foram observadas lesões uroteliais invasivas e, em geral de menor agressividade. Os níveis proteicos dos receptores toll-like (TLRs) 2 e 4, NFK-B e IL-6 foram significativamente maiores nos grupos G3 (MNU+BCG) e G5 (MNU+BCG+EFA). Uma redução significativa de VEGF associada a um aumento significativo de endostatina foram observados, em especial no grupo da associação BCG e EFA. Em contraste, os níveis protéicos de NOX1 foram reduzidos e os níveis protéicos de Nfr2 foram significativamente aumentados no grupo submetido à associação entre BCG+EFA. Portanto, a redução do grau de agressividade tumoral pela associação BCG e EFA foi provavelmente decorrente da diminuição dos TLR 2 e 4, estresse oxidativo, angiogênese e potencial aumento de apoptose na bexiga urinária de animais iniciados com a MNU. Assim, o exercício físico aeróbio poderia aumentar a eficácia do BCG no tratamento do CBNMI.
An effective therapy for Non-Muscle-Invasive Bladder Cancer (NMIBC) is the immunotherapy with BCG (Bacille Calmette-Guerin) associated with transurethral resection (TR). However, the use of BCG is associated with various adverse effects of different intensities, besides this association lead to higher tumor recurrence rate. Moreover, there are no studies showing the protective role of aerobic exercise (AE) as an adjunct therapy against the development of urothelial carcinogenesis. Thus, this study aimed to characterize and compare the morphological and molecular changes of intravesical BCG immunotherapy associated with the AE in the evolution of NMIBC chemically-induced in rats focusing on the receptors of the innate immune system, angiogenesis and oxidative balance. For inducing of BCNIM, twenty-five females Fischer 344 strain received an intravesical dose of 1.5 mg/kg of N-methyl-N-Nitrosourea (MNU) every fifteen days, a total of four applications. Five females who have not received MNU were considered as control group (Group 1) and received weekly an intravesical administration of 0.3 ml of 0.9% NaCl for six consecutive weeks. Two weeks after the last dose of MNU, the animals were divided into four groups (5 animals per group): Group MNU (Group 2) - received 0.9% NaCl for six consecutive weeks as group 1; Group MNU+BCG (Group 3) - received weekly a intravesical administration of 2 mg/ml of BCG for six consecutive weeks; Group MNU+AE (Group 4) - received the same treatment as Group 1 and 2, and simultaneously were subjected to aerobic exercise protocol (swimming), either for six consecutive weeks; and Group MNU+AE+BCG (Group 5) - received weekly an intravesical dose of BCG as described in Group 3 and simultaneously submitted to the AE protocol (swimming), both for six consecutive weeks. After treatments, urinary bladders were collected and subjected to histological and molecular analysis. The results showed that Group 2 (MNU) presented a significant reduction in body weight and more aggressive urothelial lesions when compared to the other experimental groups. In the group with BCG and the association between BCG and AE were not observed invasive urothelial lesions and its generally showed lower aggressive phenotype. The protein levels of the toll-like receptors (TLRs) 2 and 4, NF-KB and IL-6 were significantly higher in groups 3 (MNU+BCG) and 5 (MNU+BCG+AE). A significant reduction in VEGF protein associated with a significant increase of endostatin levels were observed in groups 3 (MNU+BCG) and 5 (MNU+BCG+AE) but particularly in the group 5 (MNU+BCG+AE). In contrast, a reduction in NOX 1 protein level, but an increase in Nfr2 protein level were detected in the group submitted to the association between BCG and AE treatments. Therefore, lower tumor aggressiveness for the association between BCG and AE was probably due to the decrease of the toll-like receptors 2 and 4, oxidative stress, angiogenesis and potential increase in apoptosis in the bladder urothelium of animals initiated by MNU. Thereby, aerobic exercise could increase the effectiveness of BCG in the treatment of NMIBC.
