Reatividade de marcadores de proliferação celular e de apoptose em carcinoma de células escamosas cutâneo de gatos como fatores preditivos à resposta ao tratamento com eletroquimioterapia

Resumo

O carcinoma de células escamosas (CCE) cutâneo é uma neoplasia maligna, que representa 15% dos tumores de pele em felinos. As regiões mais acometidas nos felinos pelo CCE são o plano nasal, pálpebras e pavilhões auriculares, tornando desafiadora a excisão cirúrgica com amplas margens. Desta forma, novas abordagens terapêuticas para o CCE cutâneo estão sendo investigadas, com destaque para a eletroquimioterapia (EQT). A determinação das taxas de proliferação celular e apoptose tumoral por meio da imuno-histoquímica, permite avaliar o comportamento biológico tumoral, com otimização da evolução clínica do paciente. Este trabalho teve como objetivo analisar as expressões imuno-histoquímicas de Ki-67, COX-2 e caspase-3 e correlacioná-las com os aspectos clínico-patológicos e o tipo de resposta à EQT em CCE cutâneo de felinos; assim, determinar o potencial prognóstico e/ou preditivo destas variáveis. Para tanto, foram avaliadas 13 amostras de CCE cutâneo de felinos antes da EQT e as análises estatísticas quanto à intensidade de correlação entre as variáveis foram realizadas utilizando o coeficiente de correlação de Spearman, com nível de significância de 95%. Os resultados indicam que houve correlação positiva significativa entre o grau histopatológico e o grau de pleomorfismo celular (ρ=0,81, p<0,01), correlação negativa significativa entre o grau histopatológico e a resposta à EQT (ρ= - 0,6; p=0,03), correlação positiva significativa entre o tamanho e o estadiamento tumoral (ρ=0,92; p<0,01), correlação positiva significativa entre o grau do infiltrado inflamatório e o estadiamento tumoral (ρ=0,55; p=0,048) e correlação negativa, não-significativa, entre o estadiamento tumoral e o tipo de resposta à EQT (ρ= - 0,5, p=0,08). Observou-se que não houve correlação significativa entre as imunomarcações de Ki-67, COX-2 e caspase-3 com o grau histopatológico (ρ=0,36; p=0,23/ ρ=0,036; p=0,90/ ρ=0,009; p=0,97, respectivamente). Assim como, não houve correlação significativa entre as imunoexpressões de Ki-67, COX-2 e caspase-3 com a resposta à EQT (ρ= - 0,18; p=0,54/ ρ= - 0,23; p=0,44/ ρ= - 0,12; p=0,69, respectivamente). Portanto, este estudou evidenciou que as variáveis grau histopatológico, tamanho e estadiamento tumorais, grau de pleomorfismo celular e grau do infiltrado inflamatório foram consideradas fatores prognósticos negativos para o CCE cutâneo e preditivos negativos para a resposta à EQT. Entretanto, os marcadores Ki-67, COX-2 e caspase-3 não foram considerados fatores preditivos para o tipo de resposta à EQT. Assim como não foi observada relação entre estas imunoexpressões com maior agressividade tumoral. Os CCEs avaliados neste estudo apresentaram importante marcação para COX-2, indicando um potencial alvo terapêutico. A EQT mostrou-se segura e efetiva para o controle local dos CCEs cutâneos dos felinos, porém com efetividade reduzida em lesões maiores e invasivas.
Cutaneous squamous cell carcinoma (SCC) is a malignant neoplasm, accounting for 15% of skin tumors in felines. The regions most affected in felines by SCC are the nasal plane, eyelids and auricular pavilions, making surgical excision with wide margins challenging. In this way, new therapeutic approaches for cutaneous SCC are being investigated, with emphasis on electrochemotherapy (ECT). The determination of the cellular proliferation rates and tumor apoptosis through immunohistochemistry allows the evaluation of the biological behavior of the tumor, with optimization of the clinical evolution of the patient. The aim of this study was to analyze the immunohistochemical expressions of Ki-67, COX-2 and caspase-3 and to correlate them with the clinical-pathological aspects and the type of response to ECT in feline SCC; thus determining the prognostic and / or predictive potential of these variables. In order to do so, 13 samples of cutaneous SCC of felines before the ECT were evaluated, and the statistical analyzes regarding the intensity of correlation between the variables were performed using the Spearman correlation coefficient, with a significance level of 95%. The results indicate that there was a significant positive correlation between the histopathological grade and the degree of cellular pleomorphism (ρ = 0.81, p <0.01), a significant negative correlation between the histopathological grade and the ECT response (ρ = - 0, 6, p = 0.03), significant positive correlation between tumor size and staging (ρ = 0.92, p <0.01), significant positive correlation between the degree of inflammatory infiltrate and tumor staging (ρ = 0 .55, p = 0.048) and non-significant negative correlation between tumor staging and type of response to ECT (ρ = -0.5, p = 0.08). It was observed that there was no significant correlation between the Ki-67, COX-2 and caspase-3 immunoblots with histopathological grade (ρ = 0.36, p = 0.23 / ρ = 0.036, p = 0.90 / ρ = 0.009, p = 0.97, respectively). There was no significant correlation between the Ki-67, COX-2 and caspase-3 immunoexpressions with the ECT response (ρ = -0.18, p = 0.54 / ρ = -0.23, p = 0.44 / ρ = -0.12, p = 0.69, respectively). Therefore, this study evidenced that the variables histopathological grade, tumor size and staging, degree of cellular pleomorphism and degree of inflammatory infiltrate were considered negative prognostic factors for cutaneous SCC and negative predictors for the response to ECT. However, Ki-67, COX-2 and caspase-3 markers were not considered as predictors of the type of response to the ECT. As well as no relation between these immunoexpressions with greater tumor aggressiveness was observed. The SCCs evaluated in this study presented important labeling for COX-2, indicating a potential therapeutic target. The ECT showed to be safe and effective for the local control of cutaneous SCCs of felines, but with reduced effectiveness in larger and invasive lesions.
