Efetividade clínica de sabonetes líquidos desinfetantes no controle do biofilme presente em próteses totais removíveis

Resumo

As dificuldades de higienização e controle do biofilme das próteses podem resultar em desequilíbrio da microbiota e desencadear a proliferação dos micro-organismos, inclusive patogênicos. A adoção de métodos que inativem os micro-organismos da superfície da prótese é imprescindível para prevenção e tratamento de infecções. O objetivo deste estudo clínico, randomizado do tipo cross-over foi avaliar,a efetividade de sabonetes líquidos desinfetantes na inativação de micro-organismos presentes em próteses totais removíveis. Vinte e oito participantes totalmente desdentados superiores foram selecionados para o estudo de acordo com a sequencia estabelecida para utilização dos métodos de higienização: HS: hipoclorito de sódio 0,5% (grupo controle positivo); SD: sabonete líquido Dettol; SL: sabonete líquido Lifebuoy; PBS: solução de PBS (grupo controle negativo). O tratamento consistiu na imersão das próteses totais superiores dos participantes nas soluções por 8 horas (overnight), durante sete dias. Neste estudo clínico randomizado adotou-se delineamento cruzado (crossover), onde todos os tratamentos (métodos de desinfecção) foram aplicados a todos os participantes, eliminando, assim, a possibilidade de variação entre os indivíduos em resposta a estes tratamentos. Amostras do biofilme das próteses foram obtidas antes (baseline) e após cada tratamento, utilizando-se swab estéril, e o material microbiológico foi diluído e plaqueado em meios seletivos para Candida spp. O sabonete líquido desinfetante foi considerado efetivo quando houve a redução de pelo menos 3 logs dos micro-organismos presentes nas próteses em relação ao baseline. A porcentagem de redução foi calculada pela subtração dos valores totais de UFC/mL (para cada micro-organismo) obtidos em cada tempo de avaliação em relação ao tempo zero (primeira coleta). O cálculo foi realizado para cada grupo experimental. Para a análise de variância o teste estatístico utilizado foi o ANOVA one way com correção de Welch, o pós teste de Games- Howell com nível de significância de 0,05. No presente estudo foram incluídos pacientes portadores de prótese total superior com mucosa oral e condições sistêmicas saudáveis. Foi constatada a maior incidência de C. albicans, apresentando frequência entre 66 e 88% nas intervenções analisadas, seguida de C. tropicalis, com uma frequência entre 7 e 33%. No presente estudo, hipoclorito de sódio a 0,5% foi capaz de reduzir mais de 3 logs de micro-organismos para todos os pacientes, mostrando sua alta efetividade antifúngica, tanto para espécies de Candida albicans, como para Candida tropicalis. Já o grupo PBS (controle negativo), não teve diferença significativa após as intervenções. Em relação aos grupos experimentais, ambos os sabonetes líquidos (Dettol e Lifebuoy) foram eficazes na redução dos dois tipos de micro-organismos isolados da base interna das próteses totais dos voluntários. No caso da C. albicans, as soluções tiveram a capacidade de zerar a contagem de micro-organismos ou reduzir em 3 logs de UFC/mL. Os resultados do pós-teste de Games- Howell quando os grupos foram cruzados entre si, foi possível observar que quando comparado o Grupo HS com os demais grupos de intervenções houve uma diferença significativa (p˂0,05). Já para os grupos SD e SL não foi observada diferença estatisticamente significativa entre eles. O grupo PBS apresentou diferença estatística para os demais grupos. Foi observada redução estatisticamente significante nos grupos HS (p˂ 0,001), SD (p˂ 0,001), SL (p˂ 0,001), e PBS (p=0, 643). Para Candida tropicalis, o resultado do pós-teste de Tukey quando os grupos foram cruzados entre si, pode-se observar que os valores do grupo HS e do grupo PBS, tem uma diferença estatisticamente significante (p˂0,05), também é possível observar uma diferença significativa entre o grupo controle PBS e os grupos de sabonete SD e SL . Quando comparamos os grupos HS, SD e SL não foi observado diferença significativa, comprovando assim que os três são igualmente eficazes no controle de Candida tropicalis. Até o momento, os resultados desse estudo mostram que os sabonetes líquidos poderiam ser uma alternativa para desinfecção de próteses removíveis parciais ou totais, levando em consideração sua efetividade na redução do biofilme (estudos in vivo e in vitro) e sua não citotoxicidade (estudos in vitro).

The objective of this cross-over randomized clinical trial was to evaluate the effectiveness of liquid disinfectant soaps in the inactivation of microorganisms present in total removable dentures. Twenty-eight upper fully edentulous participants were selected for the study according to the established sequence for use of the hygiene methods: HS: sodium hypochlorite 0.5% (positive control group); SD: Dettol liquid soap; SL: Lifebuoy liquid soap; PBS: PBS solution (negative control group). The treatment consisted in immersing the upper limbs of the participants in the solutions for 8 hours (overnight) for seven days. In this randomized crossover study, all treatments (disinfection methods) were applied to all participants, thus eliminating the possibility of variation among individuals in response to these treatments. Biofilm samples of the prostheses were obtained before (baseline) and after each treatment using sterile swab, and the microbiological material was diluted and plated in selective media for Candida spp. The disinfectant liquid soap was considered effective when there was a reduction of at least 3 logs of the microorganisms present in the prostheses in relation to the baseline. The percent reduction was calculated by subtracting the total values of CFU / mL (for each microorganism) obtained at each evaluation time in relation to time zero (first collection). The calculation was performed for each experimental group. For the analysis of variance the statistical test used was the ANOVA one way with Welch correction, Games-Howell post-test with a significance level of 0.05. In the present study, patients with upper total prosthesis with oral mucosa and healthy systemic conditions were included. The highest incidence of C. albicans was observed, presenting a frequency between 66 and 88% in the analyzed interventions, followed by C. tropicalis, with a frequency between 7 and 33%. In the present study, 0.5% sodium hypochlorite was able to reduce more than 3 logs of microorganisms for all patients, showing its high antifungal effectiveness for both Candida albicans and Candida tropicalis species. However, the PBS group (negative control) had no significant difference after the interventions. Regarding the experimental groups, both liquid soaps (Dettol and Lifebuoy) were effective in reducing the two types of microorganisms isolated from the internal base of the volunteers' total dentures. In the case of C. albicans, the solutions had the capacity to zero the microorganism count or reduce in 3 CFU / mL logs. The results of the Games-Howell post-test when the groups were crossed between them, it was possible to observe that when the HS Group compared with the other groups of interventions there was a significant difference (p0.05). For the SD and SL groups, no statistically significant difference was observed between them. The PBS group presented statistical difference for the other groups. A statistically significant reduction was observed in the HS (p ˂ 0.001), SD (p ˂ 0.001), SL (p ˂ 0.001), and PBS (p = 0, 643) groups. For Candida tropicalis, the Tukey post-test result when the groups were crossed with each other, it can be observed that the values of the HS group and the PBS group have a statistically significant difference (p0.05), it is also It was possible to observe a significant difference between the PBS control group and the SD and SL soap groups. When comparing the HS, SD and SL groups, no significant difference was observed, thus proving that all three are equally effective in controlling Candida tropicalis. To date, the results of this study show that liquid soaps could be an alternative for disinfection of partial or total removable dentures, taking into account their effectiveness in reducing biofilm (in vivo and in vitro studies) and their non-cytotoxicity (in vitro studies).
