Avaliação da função cardíaca pela ressonância magnética e os lipídeos séricos em fumantes jovens

Resumo

Além dos mecanismos fisiopatológicos envolvidos no aumento do risco cardiovascular, existe evidências de efeitos tóxicos direto da fumaça do cigarro no miocárdio. Assim, a utilização da ressonância magnética cardíaca constitui importante ferramenta para identificação das alterações cardíacas. O objetivo desse estudo foi avaliar a associação das características cardíacas e deposição de gordura miocárdica de fumantes jovens com os LLP, AG livres, LDL, VLDL, HDL, Colesterol total, PCR e triglicérides circulantes periféricos. No total, 57 sujeitos (29 tabagistas e 28 controles) foram incluídos no estudo. Observamos aumento no nível de lipase lipoproteica no grupo tabagista, mas não houve aumento significativo na LLP do grupo tabagista (p=0,07). Os AGL não foram estatisticamente significativos entre os grupos controle e tabagista (p=0,56). Os tabagistas apresentaram uma menor concentração estatisticamente significativa no nível de HDL no sangue (p=0,003) e maior concentração nos níveis de triglicérides (p=0,01). Também identificamos que a resistência insulínica, avaliado pelo índice de HOMA foi maior no grupo tabagista (p=0,05). Já a correlação entre a deposição de gordura e as variáveis do perfil lipídico o grupo tabagista apresentou correlação estatisticamente significativa negativa com a LLP (p=0,04). Ao analisarmos os marcadores séricos de todos os pacientes com as variáveis da RM, em relação ao VE, identificamos correlação estatística significativa positiva entre TG e volume diastólico final (VDF) (p=0,04) e SI (p=0,03). Já em relação ao VD, houve correlação estatisticamente significativa positiva entre LLP e VEI (p=0,01) e TG com VDF (p=0,03), VSF (p=0,03), Ve (p=0,04), VED (p=0,04) e VES (p=0,03). Além disso, verificamos correlação estatisticamente significativa negativa entre o diâmetro da aorta ascendente com a LLP (p=0,04) e AG (p=0,04) e TG com RA (p=0,008). O presente estudo mostrou que os tabagistas apresentaram maior resistência à insulina, redução da HDL e aumento dos TG, com correlação negativa entre os valores de TG com o diâmetro da luz da raiz da aorta e correlação positiva com a espessura do septo interventricular e com o volume diastólico final tanto do VD quanto do VE, ao passo que no ventrículo direito também foram positivas as correlações com os volumes sistólico final, de ejeção, sistólico e diastólico finais indexados. Já em relação aos AG livres, vimos correlação negativa entre seus valores e o diâmetro da luz do vaso da aorta ascendente e a LLP apresentou correlação positiva com o volume de ejeção indexado do VD e negativa com o diâmetro da luz do vaso da aorta ascendente.
Besides the pathophysiological mechanisms involved in increasing cardiovascular risk, there are evidences of direct toxic effects of cigarette smoke on the myocardium. Thus, the use of cardiac magnetic resonance is an important tool for identifying cardiac changes. The objective of this study was to evaluate the association of cardiac characteristics and myocardial fat deposition of young smokers with LLP, free AG, LDL, VLDL, HDL, total cholesterol, CRP and peripheral circulating triglycerides. This research had 57 volunteers (29 smokers and 28 controls) included. We observed an increase in the level of lipoprotein lipase in the smoker group, but not statistically significant (p = 0.07). The FFA was not statistically significant between the control and smoking groups (p = 0.56). Smokers also had a statistically significant lower concentration of HDL in the blood (p = 0.003) and a higher concentration of triglyceride levels (p = 0.01). We also found that insulin resistance, as assessed by the HOMA index, was higher in the smoking group (p = 0.05). The correlation between fat deposition and the variables of the lipid profile, the smoking group showed a statistically significant negative correlation with the LLP (p = 0.04). When analyzing the serum markers of all patients with the MRI variables, in relation to the LV, we identified a statistically significant positive correlation between TG and final diastolic volume (VDF) (p = 0.04) and SI (p = 0.03). In relation to RV, there was a statistically significant positive correlation between LLP and IEV (p = 0.01) and TG with VDF (p = 0.03), VSF (p = 0.03), Ve (p = 0.04), VED (p = 0.04) and VES (p = 0.03). In addition, we found a statistically significant negative correlation between the diameter of the ascending aorta with LLP (p = 0.04) and AG (p = 0.04) and TG with RA (p = 0.008). The present study showed that smokers had greater insulin resistance, reduced HDL and increased TG, with a negative correlation between TG values with the diameter of the aortic root lumen and a positive correlation with the interventricular septum thickness and with the final diastolic volume of both the RV and the LV, whereas in the right ventricle the correlations with the final systolic, ejection, systolic and indexed final diastolic volumes were also positive. In relation to the free GA, we saw a negative correlation between their values and the diameter of the light of the ascending aorta vessel and the LLP showed a positive correlation with the RV indexed ejection volume and a negative correlation with the diameter of the aorta vessel light ascending.
