The growing numbers of HIV patients on antiretroviral therapy (ART) failure is worrying from the point of view of public health requiring specific cares of the multiprofessional team. With the objective of evaluate the impact of pharmaceutical interventions (FI) on patients with HIV with ART failure on a Specialized Care Service has conducted a study with a convenience sample, longitudinal uncontrolled and interventional, where an FI educative and of the management of the individualized drug therapy was performed for approximately one year. In 31 patients 253 interventions were performed, 161 of these were educative, 65 management of therapy and five referrals to other health professionals. Other 11 interventions with physicians about the potential risks of drug interactions and adverse events likely. Other 11 interventions with physicians about the potential risks of drug interactions and adverse events likely. After the intervention, six patients began to regularly withdraw the drugs from the pharmacy (p = 0.0313), 12 improved adherence (p = 0.0020), on 18 the ART became effective (p = 0.0020) and 15 improved their immune status (p = 0.002). The increase of knowledge about the disease and pharmacotherapy (p = 0.0133) improved adherence, reduced the viral load and increased CD4+ count. Were no difference in general quality of life (SF - 36): functional capacity (p = 0.0017), physical aspects (p = 0.0247), pain (p = 0.0271), vitality (p = 0.0308), social functioning (p = 0.0185) and emotional (p = 0.0300) and the specific quality of life (HAT - QoL): financial worry (p = 0.0118), with the medication (p = 0.0298) and very close to statistical significance, satisfaction with life (p = 0.0538) and the sigil (0.0534). The study showed cost-effective in reducing hospital admissions, with a savings of R$ 19.43 per patient per year. The healthcare provider should take charge of the management of pharmacotherapy and the responsibility for the care of the patient must be constant. The FI with HIV patients with ART failure, for a period of one year, infected for over 10 years and on up ART to four years, provided a greater knowledge about the disease and treatment, improved adherence, reduced problems related to drugs and the negative results of the medication, promoted a greater therapeutic effectiveness, improved quality of life and reduced hospitalizations
