Uso do diamino fluoreto de prata entre concluintes do curso de odontologia, pós-graduandos e especialistas em odontopediatria: estudo observacional em nível nacional

Resumo

Dentro da vasta gama de terapêuticas indicadas para a prevenção e tratamento da cárie dentária encontra-se o diamino fluoreto de prata (DFP), que é um produto muito utilizado na paralisação de lesões de cárie e na redução de hipersensibilidade dentária. Possui algumas vantagens como ser seguro, efetivo, de baixo custo, de uso rápido, fácil e não-invasivo, que o torna um importante material a ser considerado para o atendimento odontológico, especialmente no cenário atual de pandemia da COVID-19. Entretanto, com o surgimento de novas terapêuticas para prevenção e tratamento da lesão de cárie, têm-se observado uma redução considerável no uso do DFP com o passar dos anos, mesmo diante de importantes evidências científicas apontando sua eficácia. Sendo assim, o objetivo deste trabalho é avaliar a utilização do DFP entre concluintes do curso de odontologia, pós-graduandos na área de odontopediatria, bem como especialistas na área. Trata-se de um estudo observacional, transversal, de base populacional (censo) com coleta de dados realizada por meio de questionários on-line, utilizando a ferramenta Formulários Google. Um e-mail contendo as informações sobre a pesquisa, o questionário e o encaminhamento do convite da pesquisa aos participantes foram enviados à direção de cada Instituição de Ensino Superior e aos presidentes dos Conselhos Regionais de Odontologia. Foram realizadas análises descritivas de todos os dados usando frequências absolutas e relativas e posteriormente foram estimados modelos de regressão logística entre cada variável independente e os desfechos. A partir destes modelos foram estimados os odds ratios brutos com intervalos de 95% de confiança. No total, 404 pessoas participaram deste estudo, sendo 83,7% mulheres e 16,3% homens, com idade média de 27 anos. Dos 404 participantes, 31,9% têm menos de um ano de formado e 26,2% não conhecem o DFP. Profissionais com tempo de formado entre um e 15 anos (OR=4,83; IC95%:1,31-17,77) e com mais de 15 anos de formado (OR=21,58; IC95%: 5,52-84,34), pós-graduandos, graduados e docentes (OR=10,01; IC95%: 2,06-48,76) e profissionais que trabalham em consultório odontológico (OR=7,73; IC95%: 1,52-39,30) e participantes de outras regiões (OR=2,52; IC95%: 1,34-4,73) têm maior chance de ter utilizado o DFP. Do total, 67,8% dos participantes responderam que utilizariam o DFP mesmo após resultado estético desfavorável e dentre esses observa-se maior chance de utilizá-lo pelos participantes que responderam que considerariam seu uso em situação de pandemia (OR=26,90; IC95%: 7,72-93,67), que conhecem o DFP (OR=3,39; IC95%: 2,13-5,39) e com tempo de formado entre um e 15 anos (OR=2,40; IC95%: 1,41-4,08) e com mais de 15 anos de formado (OR=2,93; IC95%: 1,68-5,11). Entre os participantes que responderam que utilizam o DFP nos atendimentos odontopediátricos, o principal motivo apontado foi que eles acreditam que o produto é eficaz (84,8%), e entre os que não utilizam o principal motivo é porque ainda não tiveram oportunidade (68,7%). Entre os professores participantes, 62% ensinam o uso do DFP nas aulas teóricas e nas clínicas. Conclui-se que embora a maioria dos participantes conheça e utilize o DFP, o aprendizado não foi durante a graduação, sendo assim, torna-se necessário que o DFP seja mais abordado nos cursos de graduação e que o mesmo seja mais difundido entre os profissionais.
Within the wide range of therapies indicated for the prevention and treatment of dental caries, is silver diamine fluoride (SDF), which is a product widely used to stop caries lesions and reduce dental hypersensitivity. It has some advantages such as being safe, effective, low cost, quick to use, easy and non-invasive, which makes it an important material to be considered for dental care, especially in the current scenario of the COVID-19 pandemic. However, with the emergence of new therapies for the prevention and treatment of caries, a considerable reduction in the use of SDF has been observed over the years, despite important scientific evidence regarding its effectiveness. Therefore, the aim of this study is to evaluate the use of the SDF among graduates of the dentistry course, postgraduate students in the field of pediatric dentistry, as well as specialists in the area. This is an observational, cross-sectional, population-based study (census) with data collection carried out through online questionnaires, using the Google Forms tool. An e-mail containing information about the research, the questionnaire and the sending of the research invitation to the participants were sent to the directors of each Higher Education Institution and to the presidents of the Regional Councils of Dentistry. Descriptive analyzes of all data were performed using absolute and relative frequencies and later logistic regression models were estimated between each independent variable and the outcomes. From these models, the brut odds ratios were estimated with 95% confidence intervals. In total, 404 people participated in this study, 83.7% women and 16.3% men, with an average age of 27 years. Of the 404 participants, 31.9% have graduated for less than one year and 26.2% do not know the SDF. Professionals who graduated between one and 15 years (OR=4.83; 95%CI: 1.31-17.77) and with more than 15 years of graduation (OR=21.58; 95%CI: 5.52- 84.34), graduate students, graduates and professors (OR=10.01; 95%CI: 2.06-48.76) and professionals working in dental offices (OR=7.73; 95%CI: 1.52 -39.30) and participants from other regions (OR=2.52; 95%CI: 1.34-4.73) are more likely to have used the SDF. Of the total, 67.8% of the participants answered that they would use the SDF even after an unfavorable aesthetic result, and among these, there is a greater chance of using it by the participants who answered that they would consider its use in a pandemic situation (OR=26.90; 95%CI: 7.72-93.67), who know the SDF (OR=3.39; 95%CI: 2.13-5.39) and who have graduated from one to 15 years (OR=2.40 ; 95%CI: 1.41-4.08) and with more than 15 years of training (OR=2.93; 95%CI: 1.68-5.11). Among the participants who answered that they use the SDF in pediatric dental care, the main reason pointed out was that they believe that the product is effective (84.8%), and among those who do not use it, the main reason is because they have not yet had the opportunity (68.7%). Among the participating professors, 62% teach the use of the SDF in theorical classes and clinics. It is concluded that although most participants know and use the SDF, the learning did not take place during graduation, therefore, it is necessary that the SDF is more addressed in undergraduate courses and that it is more widespread among professionals.
