dc.description.abstractO crescente número de pacientes portadores de HIV com falha na terapia antirretroviral (TARV) é preocupante do ponto de vista da saúde pública, necessitando da equipe multiprofissional cuidados específicos. Com o objetivo de avaliar o impacto de intervenções farmacêuticas (IF) em pacientes portadores de HIV com falha na TARV em um Serviço de Atendimento Especializado foi realizado um estudo com amostra de conveniência, longitudinal não controlado e intervencional, onde foi realizada uma IF educativa e de gestão da terapia medicamentosa individualizada, por período aproximado de um ano. Em 31 pacientes foram realizadas 253 intervenções, destas 161 foram educativas, 65 de gestão da terapia e cinco encaminhamentos para outros profissionais de saúde. Outras 11 intervenções com os médicos, sobre os potenciais riscos de interações medicamentosas e de prováveis eventos adversos. Após a intervenção, seis pacientes começaram a retirar regularmente os medicamentos na farmácia (p=0,0313), 12 melhoraram a adesão (p=0,0020), em 18 a TARV tornou-se efetiva (p=0,0020) e 15 melhoraram o seu estado imunológico (p=0,002). O aumento do conhecimento sobre a doença e a farmacoterapia (p=0,0133) melhorou a adesão, reduziu a carga viral e aumentou a contagem de CD4+. Houve diferença na qualidade de vida geral (SF36): capacidade funcional (p=0,0017), aspectos físicos (p=0,0247), dor (p=0,0271), vitalidade (p=0,0308), aspecto social (p=0,0185) e emocional (p=0,0300) e na qualidade de vida específica (HAT-QoL): preocupação financeira (p=0,0118), com a medicação (p=0,0298) e muito próximo da significância estatística, satisfação com a vida (p=0,0538) e o sigilo (0,0534). O estudo mostrou-se custo-efetivo para redução de internações hospitalares, com uma economia de R$ 19,43 por paciente por ano. O profissional de saúde deve se encarregar da gestão da farmacoterapia e a responsabilidade com o cuidado do paciente deve ser constante. A IF com pacientes portadores de HIV com falha na TARV, por período de um ano, infectados há mais de 10 anos e em TARV até quatro anos, proporcionou um maior conhecimento sobre a doença e o tratamento, melhorou a adesão, reduziu os problemas relacionados aos medicamentos e os resultados negativos da medicação, promoveu uma maior efetividade terapêutica, melhorou a qualidade de vida e reduziu as internações hospitalarespt
dc.description.abstractThe growing numbers of HIV patients on antiretroviral therapy (ART) failure is worrying from the point of view of public health requiring specific cares of the multiprofessional team. With the objective of evaluate the impact of pharmaceutical interventions (FI) on patients with HIV with ART failure on a Specialized Care Service has conducted a study with a convenience sample, longitudinal uncontrolled and interventional, where an FI educative and of the management of the individualized drug therapy was performed for approximately one year. In 31 patients 253 interventions were performed, 161 of these were educative, 65 management of therapy and five referrals to other health professionals.
