    Vigilância à saúde da criança na perspectiva dos enfermeiros da estratégia saúde da família

    This qualitative study aimed at apprehending conceptualizations and at describing experiences of nurses working in the Family Health Strategy in a medium-sized city in São Paulo state in relation to Children’s Health Surveillance, with a special focus on nursing consultation. Data were collected from 12 nurses by means of recorded semi-structured interviews that were analyzed according to the Thematic Content Analysis Method. Results were systematized into four themes: Conceptualizations about Children’s Health Surveillance; Children’s Health Surveillance in the practice of the Family Health Strategy; Nursing consultation and Children’s Health Surveillance; Proposals to qualify Children’s Health Surveillance in the realm of the Family Health Strategy. It was possible to apprehend amplified and actual conceptualizations of Children’s Health Surveillance as well as to describe experiences surrounded by difficulties to incorporate the premises of this form of Children’s Health Care in the Family Health Strategy. Nursing consultation was noteworthy as a privileged moment, but not unique, to develop such practice. Finally, based on nurses’ proposals, the importance of triggering permanent education processes targeted at these themes in family-health services was considered

    Saúde mental do idoso: percepções relacionadas com o envelhecimento em interface com o atendimento de saúde na atenção básica

    Worldwide, growth is seen in the elderly population, which has prompted the use oftechnologies of care that include all stages of life and for that use, it is necessary toprepare professionals to meet this growing demand population. This work aims to understand how old people feel by their condition and identify how old people perceive this treatment in the Family Health Unit. The research is characterized asqualitative and Minayo second analysis was used Bardin. After, thematicinterviews, it was possible to appreciate the topics: Good, dissatisfaction through the life and likes / dislikes of the service unit. This work can be seen that the elderlywho were well, allied to good living with this family, with spouses, autonomy, respect they encounter in society. And when compared with those who have a complaint, we find that lack of family and age restrictions and diseases are the main complaints

    The search for quality of life (QOL) is now an ideal among thousands of people worldwide and is currently the subject has been studied in different areas of knowledge. Thus, one can perceive it in different dimensions, each with its due importance to people's lives. The verification of quality indexes of work life (QWL) may provide information on factors that directly interfere with the satisfaction and personal motivation and collective, with reflections on the structure and excellent service. To this end, we carried out a study to know the characacteristics of quality of life of nursing professionals in a state hospital in São Paulo State, based on short range of quality of life of the World Health Organization (WHOQOL-BREF). This work is a study with a quantitative approach, cross-sectional descriptive and exploratory data analysis using descriptive statistics, involving a sample of 281 nurses who answered the scale for measuring quality of life, composed of four areas: physical, psychological, social relationships and environment. In the study, these areas were related to the professional position, and shift work. There was a satisfactory quality of life in the study population in different areas of WHOQOL-BREF. There was no statistically significant changes in quality of life among the variables. It follows that the population presented data consistent with an adequate quality of life, thus making relevant to addressing the quality of life of nursing in a healthcare organization which has as fundamental to the pursuit of quality through the accreditation process second hospital established by the National Accreditation Organization (ONA) that aims at establishing and implementing a process of improving health care, thereby stimulating the services to achieve higher standards of quality

    Enfermagem e o gerenciamento de custos: revisão sistemática nacional e internacional

    Objectives: conduct a systematic review of national and international literature, to classify the types of production, comparing the two literatures and synthesize knowledge in the area. Method: Systematic Review of Literature, in the databases LILACS, CINAHL and MEDLINE for selection of articles in their entirety, the data extracted from the articles selected were: the study of bibliographic information (title, author, journal, place of study and year of publication), and general information (multidisciplinary production or specific area, language, type of study, type of population and contributions of the study). The categories established for classification of publications were: raising the cost of procedures/ interventions, economic evaluation of specific intervention, inclusion of nursing in the economic context, using cost as an important variable in the study. Results: The study included 39 publications, with 31% of national production and the remaining 69%, internationally. Most publications were classified according to category of economic evaluation of specific intervention, followed by the category of raising the cost of procedure / intervention, which fits the majority of national production. Human resources is shaped as an important variable for obtaining cost studies that proposed to make this calculation. Conclusion: The production of literature in nursing cost management is relevant in number of publications within the proposed period and is directed to different areas within this theme. There are significant differences in national and international literature: these last use more accurate methods, have more content theorized, use more the economic evaluation tools and related more costs with a amplified context. The production of such knowledge should continue as far as possible and putting that knowledge relating the current reality, in order to amplify the field of nursing and add value

    Conhecimento dos profissionais de enfermagem sobre o Projeto Hospitais Sentinela em um hospital do interior paulista

    This study aimed to assess knowledge of nursing professionals (nurses, technicians and nursing assistants) on the Project Sentinel Hospitals in a state hospital in the interior of and identify the occurrence of under-reporting of hospital products and the reasons that prevented the team to notify them. This is a study of a descriptive and exploratory, held in a public hospital in the interior of which serves only patients of the Unified Health System (SUS). The study sample consisted of 245 nurses. Data analysis revealed that the majority of nurses and nursing assistants refers knowledge about Project Sentinel Hospitals. In relation to the Practical nurses less than half know Hospitals Project Sentry. Knowledge of the four spheres of Hospitals Project Sentinel is higher among nurses. Among the practical nurses and nursing assistants prevailing knowledge in the area of Pharmacovigilance. The nurses have more knowledge about the process of notification and are major notifiers. Technicians and nursing assistants are those who have greater interest in learning content as a whole Hospitals Project Sentry. The nurses were the professionals who witnessed the most under-and under-reporting associated with the fear, lack of knowledge on the subject, insecurity and lack of time. It can be concluded that the study identified the knowledge of nurses on Project Sentinel Hospitals and the procedure for notification of technical defects and adverse events related to health products as well as identify the presence of under-reporting among professionals. In addition, the study shows the importance and need for greater disclosure of the activities developed by Project Sentinel Hospitals among nursing professionals through meetings, lectures, brochures, among other outlets

    Percepção dos portadores de hanseníase acerca do auto cuidado

    This study aimed at evaluating the perception of leprosy patients, the disease, with regard to its concept and self-care practice, as well as their knowledge concerning drug therapy. Fifteen patients participated in the study, of whom 11 were classified as multibacillary and 04 as paucibacillary. Semi-structure interviews containing six guiding questions were used for data collection, and the Collective Subject Discourse (CSD) was utilized as a theoretical framework. Analysis was performed based on the Theory of Complexity. It showed that patients suffer pain, a fact that is reflected on the need to change habits and prevent disability. It also showed that patients have doubts and feel insecure about treatment and prognosis, and that they are also the target of social prejudice even thought the term leprosy has been replaced by Hansen‟s disease. As regards self-care, the subjects showed lack of autonomy, limitations and subjection to instructions from health care professionals, who, according to the discourse that emerged, are mostly responsible for transmitting the knowledge concerning self-care practice. This fact can be explained by evidence of self-knowledge deficit by the the subjects. Concerning polychemotherapy treatment, which is recommended by the Ministry of Health, it was observed that the knowledge concerning its action and adverse effects is precarious. This information is provided during consultation; however, CSD showed that the subjects do not apprehend it. Therefore, health care professionals must instruct their clients not only as regards conventional treatment practices, but they must also help them to know themselves and to improve their critical judgment so that they can choose the best form of living after being diagnosed with the disease, thus guiding self-care by taking such choice into account

    Informatização na educação em saúde: avaliação de site educacional

    Health education presently experiences the incorporation of information technology, thus making access to information more practical and attainable. The initiatives to make health care scientific content available virtually have shown to be useful for teaching, health care professionals and the general population. A website, however, must be constantly managed. To promote the dissemination of the website at www.educacaoemsaude.fmb.unesp.br among faculty members and undergraduate nursing students and students at Escola Técnica de Enfermagem (Vocational Nursing School) - ETE, by evaluating, among them, its graphic and functional aspects and seeking suggestions for its improvement. This is a descriptive quantitative study for which the participants were asked to fill out an instrument designed for the study. The evaluation was performed among nursing faculty members and students at Univ Estadual Paulista Júlio Mesquita Filho – UNESP and students at Escola Técnica de Enfermagem (Vocational Nursing School) – ETEC in the city of Botucatu. The number of participants totaled 88 individuals, of whom 23 were undergraduate students, 62 were students at ETEC, and 3 were faculty members. Eighty-one were females, and 7 were males. The participants’ age ranged from 17 to 56 years. Among the items that evaluated the website, the answers prevailed from “very good” to “good”. When the participants were asked what they had thought about the website, the answers were mostly positive, and the most frequently description given was “interesting”. Among the facilities, the most frequently reported was “easily understood content”, and as regards difficulties, handling the website was the most often mentioned.     Qualidade de vida e vulnerabilidade social na percepção do idoso

    According to data from the World Health Organization, the older population will grow sixteenfold from 1950 to 2025 in comparison to a fivefold population growth in the same period, which is referred to by UNO as the “Ageing Era”. This places Brazil in the sixth position in the contingent of older individuals worldwide, with a number that is higher than 32 million. Considering how topics such as quality of life and social vulnerability are important in face of the growing older population, these topics must be furthered studied so that they can be understood as important variables for both better clinical practice and scientific research. To describe the social vulnerability and evaluate the quality of life of older individuals in a population hospitalized in the internal medicine ward of Bauru State Hospital. This is a descriptive qualitative study that was conducted by means of interviews and using Bardin’s discourse analysis. The inclusion criteria used in this study were: individuals at 60 years of age who were mentally capable of answering the proposed questions. Two categories concerning Quality of Life and Social Vulnerability emerged from the interviews. The following emerged from the theme Quality of Life: “Life as something important” and subcategories that involved feeling useful in society, having a supportive family, independence, optimism and joy and survival. Also, the following emerged from the theme Social Vulnerability: “Negative recognition of older individuals in society” and subcategories that involved lack of respect, functional disability, family indifference, housing-related insecurity, an inefficient health care system and loneliness.     Percepção de mulheres sobre consulta de enfermagem, exame de papanicolaou e vulvovaginite

    The nursing consult (a job exclusive to a nurse), is a great instrument to gynecology, seeing as nurses are of fundamental importance in the prevention of uterine cancer, mostly by the use of the Papanicolau test, the most efficient method in the early detection and prevention of this kind of cancer. In addition to that, this consult includes a breast exam and possibly a vulvovaginitis exam, so that necessary forwardings and exam applications can be done. That being said, the work, in a qualitative analysis has been done in the “Healthcare center and school” in the city of Botucatu – SP, intending to measure the perception of the women in the population, of the Papanicoulau Test, the nursing consults, and their knowledge about vulvovaginitis. Twenty women were interviewed (using semi-structured interviews), and given forms after the nursing consult. The interviews were analysed using Bardin’s content analysis technique. The results show that the women’ reactions to the procedures were positive. Even Though there’s a culture of submission to medicine, and the Papanicolau test is seen by them as a necessary and important (even if unconfortable) procedure in the early prevention and detection of uterine cancer and other diseases, there was little to no knowledge about the signs, symptoms, and transmission methods for those different diseases. It is up to the nurse to facilitate these women’ access to that information, regarding not only the prevention of diseases, but also, and especially, the nursing consult’s relevance on theuterine cancer prevention program

    Avaliação da qualidade da atenção ao parto nos colegiados de gestão regional Cuesta Botucatu e Vale do Jurumirim

    The humanization of childbirth implies the understanding of this event as an important experience, and requires the redefinition of human relationships from the review of the assistance project, the understanding of pregnant women condition and human rights. To evaluate child birth assistance using a score that was developed by Botucatu Cuesta Regional Management and Jurumirim Valley Collegiates. This is an epidemiological, descriptive study that is inserted in the field of evaluation of services, programs or health projects. The data has been obtained by direct observation of deliveries, emphasizing the humane care. Results: Approximately one third of the women gave birth in a tertiary hospital (34.1%) and it was their first babies (33%).24.8% of the women received prenatal care in high-risk services. 67.1% of the births were normal, and 84.7% had no companions in the delivery room. In 47.1% of the cases the delivery was performed by obstetrician who used anesthesia in 44.7% and episiotomy in 48.2% of the deliveries. More than half of the newborns were attended by the pediatrician in the delivery room and had a delivery graph completed. Although the present study shows that 67.1% of the births were normal, caesarean rate can be considered excessive, as the WHO points out that c-sections above 15% are unlikely to be justifiable. It is important to emphasize that the Ministry of Health has to have a commitment with all women to promote safe motherhood, even in cases when the pregnancy involves a risk for both the mother and the fetus. It is noteworthy that the created score allowed us to assess variables related to the humanization of childbirth and only average and quite similar situations among the three services were evidenced. We hope that with this study, managers and professionals that work in this area can be subsidized in order to offer effective humane assistance and quality service in the delivery

    Morte e morrer: opinião dos profissionais de saúde de uma unidade de terapia intensiva pediátrica

    Presently, the dying process and death most often occur in hospitals and, particularly, in Intensive Care Units (ICU), where patients’ lives are prolonged thanks to advanced technological devices and highly efficient medicines. To learn about the opinion of health care professionals working at a Pediatric Intensive Care Unit in relation to the dying process and dying. This is a descriptive quantitative study. A questionnaire was applied to the unit’s staff members from June to August, 2011. Data were statistically analyzed. Twenty-five professionals answered the questionnaire, and 72% faced death as a natural life process. 60% felt compassion, but that feeling did not interfere with how they cared for patients. Concerning their professional training, 52% reported not to have received any concerning patients’ caregivers in the dying process or death; therefore, they experienced such situation when they were already working, and 76% reported to be interested in updating courses on that theme. Further discussion about this topic during academic education is necessary. It is also necessary to provide health care professionals with specialization courses, debates and experience exchange so that they can better understand and deal with their feelings and limitations in face of death and thus give better care to patients and relate to patients’ families during the dying process of a loved one

    Compreendendo o acolhimento em uma unidade de saúde da família na perspectiva dos usuários

    The present study aimed at understanding humanized reception at a Family Health Unit in a city in São Paulo state according to users’ perspectives. It is a qualitative investigation with a Case Study as its methodological framework and the Theory of Complexity (TC) as its theoretical basis. Data were collected from March to July, 2011 by means of non-directive interviews and participant observation, and the Flowchart that analyzes the care provision model in health care services was used. The discourses were analyzed according to Bardin’s thematic approach, from which two themes emerged: humanized reception as an act that precedes medical consultation and humanized reception as a solution to demands stemming from medical action. The study provided visibility to the forms how humanized reception is understood, that is, the moment that precedes medical consultation, being configured as a pre-consultation instance when punctual actions are performed, such as measuring vital signs, and when users are sure that they will be seen by a doctor, in addition to the perception that humanized reception is not part of the process to solve their need, since such result is achieved by means of medication dispensation permeated by the polite treatment given by professionals. These results show how the fragmented, reductionist and linear approach to caregiving is still present in the words, thoughts and culture of health service users as well as in those of health care team members. TC seems to shed light on these issues, and it may result in important improvement in the understanding of interactional relationships between team members and users concerning the work process in the Family Health model as the main strategy in Primary Care

    Acidentes na infância atendidos no pronto socorro de pediatria do Hospital das Clínicas de Botucatu que motivaram internação

    Accidents represent a major public health problem as they are frequent, preventable, and account for high morbidity and mortality rates. Children are the most vulnerable to accidents due to their inherent characteristics, and as a result of their physical, sensorial, psychomotor and cognitive limitations, which will only develop with time. Watchfulness and careful attention are of paramount importance, especially as children develop locomotor skills that are accompanied by curiosity about their surroundings. Child accidents, particularly those that could have been avoided, are the accidents most commonly seen in emergency and urgency departments. Urgency and emergency departments are the greatest allies in the attention to pediatric accidents as they offer adequate, immediate and specific high complexity care to patients at risk, viewing their vital stabilization. Investigating the causes and consequences of this insult is essential to establish a diagnosis and to contribute for the adoption of measures of prevention, control, and assistance. To identify the epidemiologic characteristics of the accidents involving children that received care at the pediatric emergency department of Botucatu Medical School Hospital and required hospitalization. This quantitative, retrospective, descriptiveanalytic epidemiologic study included all children aged 0-14 years who had had an accident and were seen at the Pediatric Emergency Department of Botucatu Medical School Hospital of São Paulo State University/UNESP between January 1/2008 and December 31/2009. A total of 227 medical charts were reviewed and 178 (78.4%) patients were included in this study. Of these, 116 (65.1%) were males and 62 (34.8%) were females. Children aged 5 - 9 years (38.9%) were the most affected, followed by those aged 10 - 14 years (37.5%). Fractures occurred in 138 (77.5%) of the cases, followed by foreign bodies...     Satisfação dos Usuários do Ambulatório Geral do Hospital das Clínicas de Botucatu

    The searches for a health service it’s a part of a process, searches for a qualified evaluation can arise the perception, in form of irritating factors taking for an alert state. The personal expectation, it makes influence in its perceptions, too. The objective was to evaluate the satisfaction degree of the users of “Seção Técnica de Ambulatório Geral do Hospital das Clínicas de Botucatu”. The work is about a quantitative and transversal study, with calculated sample of 366 users, chosen randomly and invited to participate through a questionnaire. The population was compounded by a majority of women (64,5%), between 50 to 59 years old and average degree of education (38,3%). 25 specialties were cited. The evaluate about comfort, cleaning, reception, waiting time, medical attention and nursing vary between “very good” and “good”. Obtained significant “regular” assessment, the comfort in the waiting rooms (24%), silence (25%) and cleaning bathroom (63%). Others negative evaluation was the reception about the waiting time (26%), waiting time between arrival and the consultation (34%), waiting time between the request and consultation (27%). 30,6% report to know the place for complaints and 79,4% of these pointed to the ombudsman. 62% consider their problems solved, 84,4% would indicate the hospital, 57,4% would consider the hospital better than imagined. 42,62% used the opened area for thanking, critical for staff, suggestion infrastructure improvements, cleaning and waiting time. The biggest complaint was the staff turnover in the treatment. There was disharmony between the objective and subjective questions, but, all of them must be considered to propose improvements. The satisfaction evaluation makes the service more effective, bringing credibility to the health service and the patients adhere better to the treatment

    Diagnósticos de Enfermagem Prevalentes de Pacientes em Pós-Operatório de Cirurgia Bariátrica em uma Unidade de Terapia Intensiva

    The goal of this study is to characterize the clinical outline of patients in postoperative of bariatric surgery attended in an Intensive Care Unit and identify the nursing diagnosis presented by these patients in accordance to Taxonomy II of North American Nursing Diagnosis Association (NANDA). This is a transversal, descriptive and prospective study that was desenvolved at the Intensive Care Unit of the Clinical Hospital of Botucatu Medical School – UNESP. The population was composed by all the patients who were submitted to the bariatric surgery and were attended at the Intensive Care Unit in the period between June and August of 2010, totalizing 13 patients. The data were collected based on the Nursing History that is composed by the Anamnesis, Physical Examination and Complementary Exams. The results were worked out: a) among the 13 patients studied, 10 are women (76,9%) and 3 men (23,1%); the majority is between 30 and 40 years old; 11 people of 13 are morbid obese, which means that they have Body Mass Index higher than 40 kg/m2; b) It was identified 22 nursing diagnosis and 14 of these were found in 100% of the patients. They belong to the dominions of Activity/Rest, Comfort, Elimination and Changing, Security and Protection, Functions and Relationships. Therefore, in relation to the factors, it was conclude that nursing diagnosis help nurses to understand the unsettled human answers or the risks to disturbance in an individual way, which contributes to plan specific interventions to these patients. It was possible to verify that the stage of the nursing diagnosis that is part of the nursing process should be executed by the nurses systematically, because it has been realized by the nurses’ clinical idea, but not in a systematical way

    The World Health Organization (WHO) has created a program “Safe Surgery Saves Lives”, focusing on reduce and minimize life threatening or cause serious risk to health for patients submitted to a surgery. This primary goal for this presented study is to apply the checklist from the WHO´s program for children´s surgery in a public university hospital, located in the countryside of São Paulo State, Brazil. Having the responses from professionals surgery´s team, there is an opportunity for evaluation of the features and easiest parts, which are filled in a quickly and in a easy way; focus, supporting and assistance; issues, such as lack of time for filling in, and viability of application of this protocol considering the improvement of the cross functions relationship, and quality of services, providing better assistance and support to the patient

    Regardless of their age, hospitalized children suffer from being separated from their mothers or their attachment figures. Hospitalization is a process that is difficult for children to cope with due to their separation from families, everyday environments and embedded disease scenarios. According to age, the intensity of such suffering is related to the development phase in which a child is found. The health care professionals that are close to children, not only as concerns care provision, but also emotionally, should be knowledgeable about the mother-child attachment. emotionally, should be knowledgeable about the mother-child attachment. To evaluate children’s perception of hospitalization. The study was conducted at the pediatric hospitalization unit of the Botucatu School of Medicine University Hospital – UNESP by means of interviews with hospitalized children aged 7 – 12 years and by using content analysis according to Bardin. Most of the interviewed children did not like the hospital environment and reported to miss their homes. From their statements, they showed their dissatisfaction in relation to pain and their submission to invasive procedures, which were the main complaints during the interviews. Our results agree with those found in the literature in relation to the hospital environment when the children reported the fact that they missed their homes as a difficulty to adapt. The analysis of results in this study and in previous ones led to the reflection on the importance of the presence of a relative during children’s hospitalization and the adaptation process. Despite their dissatisfaction in relation to invasive procedures, none of the children tried to stop the performance of tests and understood their importance.     With the increasing competitiveness of organizations in search of customers, it is essential that the pursuit of quality becomes a prerequisite for the survival of healthcare organization, is necessary to have working conditions that encourage both personal and professional satisfaction. This study aims to investigate the perception of nurses regarding their satisfaction in the context of work in a State Hospital and contribute to the reflection on the internal customer satisfaction as an important indicator of quality of health services that interfere with quality of care. We used the qualitative method, on phenomenology, with nurses from a state hospital that had at least one year of experience in the institution. Through interviews, it was revealed that satisfaction is related to the literature and is influenced by factors such as human resources, practice nurses, Teamwork, Perception of the patient, professional recognition, physical and material resources and rewarding work, and these themes unfold quality and satisfaction both personally and for organizations

    Infection with hepatitis C virus (HCV) is a worldwide problem of public health and who estimates 2.5% to 4.9% of infection by this virus among the population. This means that there are 3.9 to 7.6 million people at risk of developing cirrhosis or liver cancer. In Brazil, 20% to 58% of patients with chronic liver disease have antibodies to HCV (anti-HCV). To characterize the profile of patients undergoing treatment for hepatitis C in the Ambulatory General HC-FMB/UNESP, identify aspects of the disease and the phases of nursing process addressed during consultation. Transverse and descriptive study involving 38 patients undergoing treatment for Hepatitis C in Ambulatory General Area (Viral Hepatitis) in the period from July to September 2010. The population consisted of 38 patients, most of the males with completed higher education level, Catholic, married and aged predominantly between 41 and 60 years. Among the drugs used, we find the use of antihypertensive, antidepressant / anxiolytic and antidiabetic / hypoglycemic. With respect to specific medications used to treat hepatitis C, we found the use mainly of alfapeguinterferona 2b + ribavirin. The drugs used were complementary erythropoietin and filgastrim. There was a predominance of fibrosis 2 (F2) and genotype 1 (G1). Regarding the means of contamination, it was stressed blood transfusion and injection drug use. The most frequent drug reactions were decreased appetite, weight loss and discouragement. : The Nursing Process is considered a valuable tool in caring for patients with hepatitis C, because it works as identifying aspects of lifestyle, needs and potential of these patients and allows the deployment of humanized care strategies aimed at reduction of health hazards and improving the quality of life of these patients

    Dramatic changes occur in the lives of patients on peritoneal dialysis and his family life, changing lifestyle, professional and social activities. Considering the important role of family in treatment, is crucial for nurses to know how these families perceive their role as caregivers and their difficulties and needs, in the process of peritoneal dialysis. The approach chosen for the study was the phenomenological method, based on Merleau-Ponty, who proposes to understand the human experience, from the description of those who lived it. The study findings show that in the beginning of treatment, family members, feel impacted with the disease severity and eager to become responsible for maintenance treatment, fearing not match the expectations.Guided by staff, appear confident in taking care and deal with the difficulties and complications of treatment, supported by professionals. However, resent the great changes in their social activities and work, his life turns out to be quite limited, due to the dedication to a sick family member. Many feel overwhelmed because they are not supported by other family members. Some envision a future outlook for renal transplant, others seem skeptical, given the long waiting list, especially when advanced age of the sick family member. These results suggest the need for individualized attention to family caregivers, and to encourage the family to organize itself and develop a joint work. In this sense, the design of health care, taking care with the focus of the family, it seems highly appropriate in preparing the plan of family orientation, this is defined as a dynamic unity, which, working in harmony, can contribute positively in the treatment of health a sick member
