Abstract (portuguese)

O objetivo deste estudo foi avaliar o efeito de diferentes medicações intracanal no volume e reparo ósseo de dentes com lesão periapical tratados endodonticamente. Para isso, foram selecionados 42 dentes unirradiculares de pacientes com infecção endodôntica primária e periodontite apical, e divididos em 3 grupos (n=14) de acordo com a medicação intracanal (MIC): Hidróxido de Cálcio + Clorexidina gel 2%; Ultracal XS® e NAC (N-Acetilcisteína) + Solução Salina Fisiológica. Para realizar a seleção dos dentes, foram obtidas radiografias periapicais (RP) e tomografias computadorizadas de feixe cônico (TCFC) antes de iniciar o tratamento endodôntico (T0). Os dentes foram preparados utilizando sistema reciprocante de lima única, irrigados com hipoclorito de sódio 2,5%, receberam a MIC e a obturação foi realizada após o período de 14 dias. Após a obturação, os dentes foram restaurados com resina composta e reavaliados com uma nova tomografia após o período de 6 meses (T6). Os arquivos das tomografias foram exportados no formato DICOM (Digital Imaging and Communications in Medicine) para avaliar o volume das lesões antes e após o tratamento endodôntico pelo processo de segmentação semiautomático nos períodos pré-estabelecidos (T0 e T6). Os dados obtidos foram analisados por testes estatísticos descritivos e inferenciais com significância de 5% (p<0,05). Pôde-se concluir que as medicações intracanal utilizadas apresentaram bom desempenho clínico, favorecendo o processo de reparo ósseo periapical, sem diferenças estatísticas significantes entre elas. A NAC pode ser uma alternativa promissora como medicação intracanal, mas outros estudos clínicos ainda são necessários

Abstract (english)

The objective of this study was to evaluate the effect of different intracanal medications on the volume and bone repair of teeth with periapical lesions treated endodontically. To this end, 42 single-rooted teeth from patients with primary endodontic infection and apical periodontitis were selected and divided into three groups (n=14) according to the intracanal medication (ICM): Calcium Hydroxide + 2% Chlorhexidine gel; Ultracal XS®; and NAC (N-Acetylcysteine) + Physiological Saline Solution. To select the teeth, periapical radiographs (PR) and cone-beam computed tomography (CBCT) scans were obtained before starting the endodontic treatment (T0). The teeth were prepared using a rotary system, irrigated with 2.5% sodium hypochlorite, received the ICM, and obturation was performed after a 14-day period. After obturation, the teeth were restored with composite resin and re-evaluated with a new tomography scan after six months (T6). The tomography files were exported in DICOM format (Digital Imaging and Communications in Medicine) to evaluate the lesion volume before and after endodontic treatment through a semi-automated segmentation process at the preestablished time points (T0 and T6). The data obtained were analyzed using descriptive and inferential statistical tests with a significant level of 5% (p<0.05). It was concluded that the intracanal medications used showed good clinical performance, promoting the periapical bone repair process, with no statistically significant differences among them. NAC may be a promising alternative as an intracanal medication, but further clinical studies are still needed
