Placa interoclusal com dispositivo vibratório comparada com a de superfície oclusal plana em pacientes com bruxismo

Abstract

A proposta do presente estudo foi avaliar o efeito da terapia com diferentes tipos de placas interoclusais em pacientes com bruxismo do sono através da eletromiografia (EMG) computarizada dos músculos masseter e temporal (feixe anterior). Foram selecionados oito participantes de ambos os sexos, com idade entre 18 e 28 anos (idade média 22 anos) e com sinais e sintomas clínicos evidentes de bruxismo. Os pacientes selecionados foram divididos aleatoriamente em dois grupos. Utilizamos dois tipos de placas ambas confeccionadas para a arcada superior: a de superfície oclusal plana e uma de nossa autoria que denominamos placa interoclusal com dispositivo vibratório. O primeiro grupo recebeu a placa interoclusal com dispositivo vibratório, enquanto o segundo grupo recebeu a placa interoclusal de superfície oclusal plana. Os dois grupos de pacientes foram submetidos a avaliações eletromiográficas dos músculos masseter e temporal (feixe anterior) bilateralmente, antes e após 21 dias de uso das diferentes placas interoclusais. A atividade eletromiográfica foi registrada com os paciente em posição de repouso mandibular e em contração isométrica dos músculos masseter e temporal. Os resultados revelaram que: na posição de repouso, para os músculos masseter e temporal anterior, houve uma redução significativa da atividade eletromiográfica durante o período de tratamento tanto para o primeiro grupo quanto para o segundo grupo. Durante o fechamento da mandíbula, para os músculos masseter e temporal houve reduções significativas da atividade eletromiográfica. Entretanto, não há diferença estatisticamente significativa entre a utilização da placa com dispositivo vibratório e a de superfície oclusal plana, sendo os dois tipos de placas eficientes para o tratamento do bruxismo do sono

The purpose of this study was to evaluate the effectiveness of different type of interoclusal splints in patients who suffer from bruxism while sleep, through computarized electromyography. Eight patients were selected, of both sexes, ranging from 18 to 28 of age (with the average age bein 22 years old), with evident signs and symptoms of bruxism. The selected patients were randomly divided in two groups. Two types of splints were used, both of which were made for the upper arch: the interocclusal plane surface and one created by our team, which is called the interocclusal splint with a vibrating device. The first group was treated with the interocclusal splint with a vibrating device; while the second group was treated with the interocclusal plane surface. The two groups of patients were submitted to electromyographic evaluations of the masseter and anterior temporal muscles on both sides. The evaluations were made at the beginning of the treatment and after 21 days of use of the two types of splints. The collected information of the electromyography was done in resting position and with isometric contractors from masseter and temporal muscles. The collected informations revelead that: the masseter and anterior temporal muscles, in the rest position, had an expressive reduction in electromyographic activity, in the first and second group, during the treatment. The same fact occuried for the masseter and anterior temporal muscles, during movements of closing the mandible. However, ther were no statistical differences between the tratment with the interocclusal splint with a vibrating device and the treated with the interocclusal plane surface; both of them were consedered effectiveness for patients who suffer from bruxism while sleep
