Extensive necrosis in the oral mucosa of an epilepsy patient: a clinical report and literature review

Abstract

Pacientes com epilepsia frequentemente apresentam lesões orais e maxilofaciais como traumatismos dentários, lacerações de tecidos moles e fraturas de ossos da face. No entanto, a necrose tecidual da mucosa bucal ainda não foi relatada entre esses pacientes. O objetivo deste artigo é relatar um caso incomum de necrose pós-traumática da mucosa bucal decorrente de uma crise epiléptica. Também foi realizada uma revisão da literatura sobre lesões orais em pacientes com epilepsia. Um homem de 55 anos com epilepsia recorrente foi encaminhado para avaliação de uma lesão extensa na mucosa bucal com duas semanas de evolução. O paciente relatou que havia mordido a região durante uma crise epiléptica. O exame oral revelou área necrótica branco-acinzentada indolor na região posterior da mucosa jugal do lado esquerdo medindo cerca de 3 × 2 cm. Os achados clínicos levaram ao diagnóstico de necrose tecidual, e o desbridamento cirúrgico foi então realizado. Um acompanhamento de dois anos demonstrou cicatrização completa associada à formação de tecido fibroso. A necrose tecidual pode ocorrer na mucosa bucal de pacientes com epilepsia e deve ser incluída na gama de lesões orais e maxilofaciais associadas a crises epilépticas recorrentes.

Epilepsy patients often present oral and maxillofacial injuries such as dental trauma, soft tissue lacerations, and fracture of facial bones. However, oral tissue necrosis has not been reported yet among these patients. The aim of this article is to report an unusual case of post-traumatic necrosis in the oral mucosa resulting from a recurrent seizure. A review of the literature on oral injuries in epilepsy patients was also performed. A 55-year-old man with recurrent epilepsy was referred for evaluation of an extensive lesion in the buccal mucosa with two weeks of evolution. The patient reported that he had bitten the region during an epileptic seizure. Oral examination revealed a painless grayish-white necrotic area in the posterior region of buccal mucosa on the left side measuring about 3×2 cm. Clinical findings led to the diagnosis of tissue necrosis, and the surgical debridement was then performed. A two-year follow-up demonstrated complete healing associated with the formation of fibrous tissue. Tissue necrosis may occur in the oral mucosa of epilepsy patients and should be included in the range of oral and maxillofacial injuries associated with recurrent seizures.
