A osteonecrose dos maxilares relacionada a medicamentos (MRONJ) é uma condição na qual ocorre a degradação e morte de células ósseas devido à diminuição da reabsorção óssea e pode ocorrer como consequência do uso de antirreabsortivos, como bisfosfonatos e denosumabes. O objetivo desse trabalho foi relatar o caso de um paciente, do sexo masculino, 73 anos, diagnosticado com câncer de próstata apresentando metástase óssea em região de quadril o qual manifestou MRONJ após uso de denosumabe. Procurou o Projeto Onco, pois havia instalado implante dentário em região mandibular, o qual apresentou falha na reparação tecidual. Diante dessa situação, foram realizadas 22 sessões de terapia fotodinâmica (PDT), com laser vermelho e fotossensibilizador azul de metileno a 0,01%, associada à antibioticoterapia e bochecho com digluconato de clorexidina a 0,12%, previamente à excisão cirúrgica da lesão necrótica. A PDT se manteve no trans e pós operatório e após 15 dias da cirurgia foi observado a reepitelização inicial do tecido. Conclui-se que a PDT pré,trans e pós operatória associada a antibioticoterapia, com orientação de higiene e excisão cirúrgica foram eficientes à preservação tecidual e promoveu cicatrização da MRONJ
Abstract (english)
Medication-Related Osteonecrosis of the Jaws (MRONJ) is a condition in which the degradation and death of bone cells occur due to decreased bone resorption and may result from the use of antiresorptive drugs, such as bisphosphonates and denosumab. The aim of this study was to report the case of a 73-year-old male patient diagnosed with prostate cancer with bone metastasis in the hip region, who developed MRONJ after using denosumab. He sought the Onco Project, as he had undergone dental implant surgery in the mandibular region, which showed tissue repair failure. In this situation, 22 sessions of photodynamic therapy (PDT) were performed, with red laser and 0.01% methylene blue photosensitizer, associated with antibiotic therapy and mouthwash with 0.12% chlorhexidine digluconate, prior to surgical excision of the necrotic lesion. PDT was maintained in the trans and postoperative period, and after 15 days from the surgery, the initial reepithelialization of the tissue was observed. It is concluded that pre, trans, and postoperative PDT associated with antibiotic therapy, hygiene guidance, and surgical excision were efficient for tissue preservation and promoted healing of MRONJ
