Reabilitação oral mandibular por meio de uma overdenture barra-clipe com o'ring

Abstract

Pacientes que apresentam perda total dos elementos dentários ficam sujeitos a inúmeras complicações relacionadas a função mastigatória, social e estética. Desse modo, existem diversos tratamentos para a correção desses problemas que acometem principalmente pessoas mais idosas. A overdenture é um tipo de prótese total removível implantomucossuportada, indicada nos casos de reabilitação oral onde há extrema reabsorção óssea do rebordo residual, cuja solução é a instalação de dois ou mais implantes, preferencialmente na região de caninos ou primeiros pré-molares, a fim de se obter maior retenção e estabilidade para a recuperação da função mastigatória, estética e contorno facial. Sendo um tratamento de custo menos elevado em relação a próteses totais fixas por implantes, e, com grande benefício ao órgão mastigatório. Este caso clinico tem como objetivo analisar a confecção de uma overdenture mandibular, abrangendo temas relacionados ao planejamento do tratamento e protocolo protético na seleção do sistema de retenção, no caso, apresentando barra-clipe com o'ring, contribuindo para um balanceio articular correto, melhor distribuição de forças mastigatórias, estabilização e prevenção do deslocamento horizontal da overdenture, visando o melhor resultado de acordo com as características morfológicas e biológicas do paciente. Os resultados mostraram melhora na função mastigatória e estética do paciente, sendo uma alternativa mais econômica e funcional para reabilitação oral.

Patients who experience total loss of dental elements are subject to numerous complications related to masticatory, social, and aesthetic functions. Thus, various treatments exist to correct these issues, which primarily affect older individuals. The overdenture is a type of removable implant-supported prosthesis, indicated in cases of oral rehabilitation where there is extreme bone resorption of the residual ridge. The solution involves the installation of two or more implants, preferably in the canine or first premolar region, to achieve greater retention and stability for the recovery of masticatory function, aesthetics, and facial contour. Being a more cost-effective treatment compared to fixed implant-supported total prostheses, it offers significant benefits to the masticatory system. This clinical case aims to analyze the fabrication of a mandibular overdenture, covering topics related to treatment planning and prosthetic protocol in the selection of the retention system. In this case, a bar-clip with O-ring was used, contributing to proper joint balance, better distribution of masticatory forces, stabilization, and prevention of horizontal displacement of the overdenture, ensuring the best outcome according to the patient’s morphological and biological characteristics. The results showed an improvement in the patient's masticatory function and aesthetics, making it a more economical and functional alternative for oral rehabilitation.
