Tratamento de sequela de trauma em mandibula com auxilio do planejamento digital

Abstract

Em diversos casos da cirurgia e traumatologia bucomaxilofacial precisamos recorrer a técnicas auxiliares como as oferecidas pela odontologia digital. Este caso clínico, visa relatar um acidente motociclístico ocorrido no do município de Araçatuba - SP e encaminhado aos cuidados da equipe de cirurgia e traumatologia bucomaxilofacial. Prontamente ao ocorrido, houve a análise dos exames de imagem, sendo assim diagnosticado as fraturas mandibulares em ângulo mandibular do lado direito e parassínfise mandibular do lado esquerdo, bem como a fratura em vertebra cervical C2. Logo, como plano de tratamento, foi proposta uma primeira resolução da fratura cervical por meio do colete cervical, devido a indicação da equipe de neurocirurgia e aos riscos ao paciente. Em um segundo momento, após o período cicatricial (90 dias) da fratura cervical e retirada do colete cervical, foi iniciado as intervenções necessárias para resolução da fratura mandibular, que não foram reparadas de maneira adequada deixando sequelas a alteração da oclusão do paciente, sendo necessário a intervenção cirúrgica. Para resolução do caso foi realizado um planejamento cirúrgico e virtual por meio do software Ortog On Blonder, o qual mimetizou a fratura inicial e tornou possível a confecção de guias de plano de corte das posições iniciais da fratura na região de ângulo mandibular direito e parassínfise/corpo do lado esquerdo, permitindo assim que a fratura fosse estabilizada, de forma ideal, com placas de titânio e devolvida a oclusão correta ao paciente. Este caso, mostra a importância da odontologia digital para resoluções de casos mais complexos, em que se deve priorizar outras intervenções a saúde, garantindo um resultado de excelência em situações complexas.

In many cases of oral and maxillofacial surgery and trauma, we need to resort to auxiliary techniques such as those offered by digital dentistry. This clinical case reports a motorcycle accident that occurred in the city of Araçatuba, São Paulo, and was referred to the oral and maxillofacial surgery and trauma team. Imaging tests were analyzed immediately after the incident, diagnosing mandibular fractures in the right mandibular angle and left mandibular parasymphysis, as well as a fracture in the C2 cervical vertebra. Therefore, the treatment plan proposed an initial cervical fracture repair using a cervical brace, based on the neurosurgery team's recommendation and the risks to the patient. Subsequently, after the cervical fracture had healed (90 days) and the cervical brace was removed, the necessary interventions were initiated to repair the mandibular fracture. These were not repaired adequately, resulting in altered occlusion, necessitating surgical intervention. To resolve the case, virtual surgical planning was performed using the Ortog On Blonder software. This software mimicked the initial fracture and enabled the creation of cutting plane guides for the initial fracture positions in the right mandibular angle and left parasymphysis/body region. This allowed the fracture to be optimally stabilized with titanium plates and restored to correct occlusion. This case demonstrates the importance of digital dentistry in resolving more complex cases, in which other health interventions must be prioritized, ensuring excellent results in complex situations.
