Hipermobilidade articular generalizada e sua implicação na articulação temporomandibular e músculos mastigatórios: relato de caso

Abstract

A presença de alterações ou deficiências nas estruturas ligamentares resulta em uma série de alterações que compõem as Desordens Hereditárias do Tecido Conectivo, sendo a Síndrome da Hipermobilidade Generalizada (HAG) citada como a mais comum entre elas. Sendo assim, a Articulação Temporomandibular (ATM) também pode se apresentar hipermóvel e impactar em Distúrbios Temporomandibulares (DTM), na disfunção musculoesquelética e vice-versa. Objetivos: Apresentar o relato de um caso clínico de paciente portadora de HAG e hipermobilidade da ATM, e discutir suas implicações no sistema estomatognático e suas funções. Materiais e métodos: Anamnese, Exame Físico e Análise de Exames de Imagem complementares. Conclusão: Conclui-se que a HAG pode influenciar na Hipermobilidade Condilar e aparecimento de DTMs, se unindo a vários outros fatores que podem engatilhar os sintomas dolorosos de diversas condições que se associam e colaborar com a evolução do quadro da paciente, principalmente na presença de episódios de estresse e ansiedade frequentes. Dessa forma preconiza- se uma visão integral do paciente ao investigar o diagnóstico e suas possíveis causas, além de aconselhar tratamento multidisciplinar incluindo por exemplo, uma equipe multiprofissional.

The presence of alterations or deficiencies in the ligament structures results in a series of alterations that make up the Hereditary Connective Tissue Disorders, with the Generalized Hypermobility Syndrome (GAH) cited as the most common among them. Therefore, the Temporomandibular Joint (TMJ) can also be hypermobile and impact Temporomandibular Disorders (TMD), musculoskeletal dysfunction and vice versa. Objectives: To present the report of a clinical case of a patient with HAG and TMJ hypermobility, and discuss its implications on the stomatognathic system and its functions. Materials and methods: Anamnesis, Physical Examination and Analysis of Complementary Imaging Examinations. Conclusion: It is concluded that HAG can influence condylar hypermobility and the appearance of TMDs, joining several other factors that can trigger the painful symptoms of several conditions that are associated and collaborate with the evolution of the patient's condition, especially in the presence of frequent episodes of stress and anxiety. Thus, a comprehensive view of the patient is recommended when investigating the diagnosis and its possible causes, in addition to advising multidisciplinary treatment, including, for example, a multidisciplinary team.
