Tratamento da mordida aberta anterior dentoalveolar na dentição mista

Abstract

A mordida aberta (MA) é uma má oclusão no sentido vertical caracterizada pela presença do trespasse vertical negativo, em que as bordas incisais dos incisivos superiores não sobrepõem os incisivos inferiores. A MA pode ser de natureza esquelética ou dentoalveolar. A mordida aberta anterior esquelética (MAAE) ocorre quando está presente um padrão de crescimento craniofacial hiperdivergente, com o ângulo do corpo da mandíbula maior que 130o. Dessa forma, a MA é ampla, estendendo-se até primeiros ou segundos molares decíduos, sendo, portanto, secundária ao padrão esquelético. A mordida aberta anterior dentoalveolar (MAAD) tem como característica o trespasse vertical negativo circunscrito à área entre caninos de formato ovalado horizontal, geralmente relacionada com hábitos de sucção não nutritivos e com padrão de crescimento craniofacial equilibrado. O tratamento da MAAD na dentição mista tem apresentado resultados favoráveis e são descritos vários métodos de tratamento. Entre estes métodos estão aparelhos removíveis como a placa de acrílico com grade palatina e aparelhos fixos (cimentados) como o bihélice e o quadrihélice com grade, esporões linguais, entre outros. O objetivo deste trabalho é apresentar, por meio de uma revisão de literatura, alguns protocolos de tratamento dessa má oclusão e relatar dois casos clínicos: um tratado com a placa de acrílico com grade palatina e outro só com grade.

Open bite (OB) is a vertical malocclusion characterized by a negative overbite, in which the incisal edges of the maxillary incisors don’t overlap those of the mandibular incisors. OB may have either a skeletal or dentoalveolar origin. The skeletal anterior open bite (SAOB) occurs in individuals with a hyperdivergent craniofacial growth pattern, where the mandibular plane angle exceeds 130°. In such cases, the open bite is extensive, often reaching the first or second deciduous molars, and is therefore secondary to the skeletal pattern. The dentoalveolar anterior open bite (DAOB), in contrast, is characterized by a negative vertical overlap restricted to the area between the canines, with an oval-shaped horizontal contour. It is usually associated with non- nutritive sucking habits and a balanced craniofacial growth pattern. Treatment of DAOB in the mixed dentition has shown favorable outcomes, and several therapeutic approaches have been described. These include removable appliances, such as the acrylic plate with palatal crib, and fixed (cemented) appliances, such as the bi-helix and quad-helix with crib, lingual spurs, among others. The aim of this study is to present, through a literature review, several treatment protocols for this malocclusion and to report two clinical cases: one treated with an acrylic plate with palatal crib and another with only crib.
