Clareamento interno e restauração com resina composta universal: abordagem integrada e simplificada

Abstract

A associação de clareamento dental e restaurações em resina composta é uma prática comum na odontologia estética, geralmente realizada após o clareamento para harmonizar a cor da restauração com a estrutura dental. No entanto, o surgimento de uma resina composta universal que se adapta à cor do dente após o clareamento elimina a necessidade de substituição da restauração, simplificando o tratamento. O objetivo deste relato de caso é descrever o protocolo clínico da associação entre a utilização de uma resina composta universal no tratamento estético de um incisivo central escurecido e o clareamento interno. Para tanto, inicialmente, foram realizadas a troca das restaurações nos dentes 11 e 12, seguindo o protocolo restaurador que consiste em isolamento absoluto, remoção das restaurações insatisfatórias, profilaxia com pedra pomes e água, condicionamento ácido, aplicação do sistema adesivo, fotopolimerização e inserção da resina composta Universal com auxílio de cunha de madeira e tira de poliéster. Após a fotoativação final, realizou-se o acabamento e polimento com discos e polidores diamantados. Em seguida, na mesma sessão, para o clareamento interno realizou-se o acesso a câmara pulpar, desobturação da câmarapulpar, confecção do plug cervical com cimento de Ionômero de vidro, e inserção do peróxido de hidrogênio a 35% no interior da câmara pulpar e restauração provisória. O gel clareador foi substituído semanalmente durante quatro semanas e no final foi realizado uma sessão de consultório com peróxido de hidrogênio a 40%. Após o período de clareamento, a câmara pulpar foi preenchida com hidróxido de cálcio por 14 dias antes da restauração definitiva com ionômero de vidro e resina composta.

The combination of tooth whitening and composite resin restorations is a common practice in esthetic dentistry, typically performed after bleaching to harmonize the color of the restoration with the tooth structure. However, the emergence of a universal composite resin that adapts to the tooth color after bleaching eliminates the need for restoration replacement, thus simplifying the treatment and allowing it to be performed prior to the whitening procedure. The aim of this case report is to describe the clinical protocol involving the use of a universal composite resin in the esthetic treatment of a discolored central incisor in combination with internal bleaching. Initially, the restorations on teeth 11 and 12 were replaced, following a restorative protocol that included rubber dam isolation, removal of unsatisfactory restorations, prophylaxis with pumice and water, acid etching, application of the adhesive system, light curing, and insertion of the universal composite resin using a wooden wedge and polyester strip. After final light curing, the restoration was finished and polished with discs and diamond polishers. In the same appointment, internal bleaching began with the initial shade recorded as A3.5, followed by access to the pulp chamber, removal of the root canal filling material, placement of a cervical plug using glass ionomer cement, insertion of 35% hydrogen peroxide into the pulp chamber, and provisional restoration. The bleaching gel was replaced weekly for four weeks, and a final in-office bleaching session was performed using 40% hydrogen peroxide. After the bleaching period, the pulp chamber was filled with calcium hydroxide for 14 days before the final restoration with glass ionomer cement and composite resin. At the end of the treatment, the tooth color had changed to closely match the adjacent teeth, with a final recorded shade of A2. Thus, it can be concluded that the use of a universal composite resin simplifies the restorative protocol when combined with tooth whitening, eliminating the need to replace the restoration after bleaching.
