Tratamento cirúrgico de queratocisto odontogênico mandibular recidivante: relato de caso clínico

Abstract

O queratocisto odontogênico (QO) dos maxilares têm sido uma das entidades patológicas mais controversas na patologia maxilofacial, em grande parte devido ao potencial de recorrência. O presente estudo tem por objetivo relatar um caso clínico de uma paciente de 18 anos de idade, que apresentava uma lesão em região de ramo, ângulo e corpo mandibular do lado esquerdo, possuindo edema pronunciado na região, fístula e incomodo, obtendo-se o diagnóstico de queratocisto odontogênico. A paciente foi submetida à drenagem cirúrgica, curetagem da lesão e exodontia do dente 38, sob anestesia geral. Após quatro anos sem comparecer a clínica, a paciente voltou e a lesão apresentava-se maior, acometendo novas regiões mandibulares. Foi instalado um dispositivo para descompressão e após 1 ano e 9 meses foi realizada enucleação da lesão queratocística recidivante. Ela está sendo acompanhada clínica e radiograficamente até os dias atuais, havendo possível recidiva do cisto, porém em menor magnitude. Com isso, concluímos que o diagnóstico deste cisto necessita, mais do que nunca, da união dos achados clínicos, radiográficos e histopatológicos propriamente ditos, além do que as formas de tratamento ainda são muito discutidas na literatura, por ser uma entidade patológica com elevado índice de recorrência.

Odontogenic keratocystic (OK) of the jaws has been one of the most controversial pathological entities in maxillofacial pathology, largely due to the potential for recurrence. The present study aims to report a clinical case of an 18 year old patient, who had a lesion in the region of the ramus, angle and mandibular body on the left side with great swelling in the region, fistula and discomfort that resulted a diagnosis of odontogenic keratocyst. This was submitted to drainage of the abscess, curettage of the lesion and extraction of tooth 38, under general anesthesia. The patient returned after four years without attending the clinic and the lesion was larger, affecting new mandibular regions. A decompression device was installed and after 1 year and 9 months enucleation of the recurrent keratocystic lesion was performed. She is being followed up clinically and radiographically until the present day with a possible recurrence of the cist but in a smaller size. That´s why we conclude that the diagnosis of this cist requires, more than ever, the union of clinical, radiographic and histopathological findings themselves, in addition to the forms of treatment that are still much discussed in the literature, as it is a pathological entity with a high rate. of recurrence.
