    A perspective on the proposal for neurocognitive disorder criteria in DSM-5 as applied to HIV-associated neurocognitive disorders

    HIV-associated neurocognitive disorders remain common in the current era of effective antiretroviral therapy. However, the severity at presentation of these disorders has been reduced, and the typical manifestations have changed. A revision of the American Academy of Neurology (AAN) criteria has been made on this basis, and a revision of the analogous criteria by the American Psychiatric Association will be forthcoming in the Diagnostic and Statistical Manual of Mental Disorders (DSM)-5. This article compares the relevant sets of diagnostic criteria that will be employed. It is concluded that a greater degree of integration of the revised, HIV-specific AAN criteria for HIV-associated neurocognitive disorders with the criteria proposed for the DSM-5 would prove advantageous for research, clinical, educational and administrative purposes. © 2011 Future Medicine Ltd.

    Assombrando o arquivo: a Biblioteca Nacional e o homoerotismo na Coleção Alair Gomes

    A partir da compreensão de que o registro e a preservação da história dos sujeitos dissidentes das normativas de gênero e sexualidade são um direito epistêmico, neste artigo busco apontar algumas notas de uma cartografia que realizei na Coleção Alair Gomes, da Biblioteca Nacional. Alair Gomes (1921-1992) é considerado o precursor do homoerotismo na fotografia brasileira. Criada entre as décadas de 1960 a 1980, sua obra vai desde as imagens do corpo masculino jovem seminu nas praias de Ipanema, trabalho pelo qual ele é mais conhecido, até as fotografias de práticas esportivas, carnaval e botânica. Além do conjunto de 16 mil fotografias e 150 mil negativos, a Coleção Alair Gomes é composta por manuscritos relacionados às suas atividades acadêmicas, artísticas e diários íntimos. Trata-se aqui de indagar como uma instituição pública da memória, como a Biblioteca Nacional, se relaciona com esse tipo de arquivo.

    Draw-A-Person with A Story (DAP-S) Test and Children with Learning Difficulties

    Learning disabilities (LD) represent a challenge for the educational, social and health fields. The aim was to evaluate the applicability of the Draw-A-Person with a Story (DAP-S) test considering the understanding of children with learning disabilities. Twenty children from the third grade of a Public Elementary School in São Paulo-SP, identified by the school coordination as suffering some type of LD were individually evaluated. Their drawings were analyzed according to the emotional indicators (EI) and maturational indicators (MI) proposed by Koppitz and also through the free inspection method of the projective (graphic/narrative) material. The mean for the MI was lower, and the mean for the EI was higher than the result expected for the age. The correlation between the indicators was inverse and significant, although moderate. The DAP-S contributed to a singular understanding of children with LD and made it possible to raise hypotheses about the reasons for their difficulties, clarifying the data provided by the MI and EI.

    Homofobia familiar: Abrindo o armário 'entre quatro paredes'

    Sexuality device authorizes families to become the guardians of its members' sexuality, ensuring the exclusion of any kind of dissidence from heteronormativity by means of reinforcement of homophobia. This paper presents the analyses of female participants who are dissident of heteronormativity. They were accessed through snow-ball technique and were interviewed. After transcribing their interviews, the researcher produced narratives of their lives with were corroborated with the participants. Data were analyzed from a poststructuralist perspective. By then, we privileged the study of the manifestation of homophobia in the family, developing a genealogy of their detractors effects, but also highlighting modes of resistance to it. This locus of manifestation of homophobia, obscured by the intimacy of the private world, is still a fairly discussed topic in national studies and maximizes the damage caused by discrimination in macro social spaces. We pointed out, then, family as an important target of action for public policies which aim the protection of human and civil rights, as well as all kinds of violence and discrimination.

    Considerations on the diffusion of Kleinian thought in Brazil

    This article describes the different moments that characterized the diffusion of Kleinian thought in psychoanalytic societies in Brazil. The article results from qualitative historical research based on interviews with thirteen psychoanalysts who participated in this process of diffusion. The first influences appeared in 1950, exercised by pioneers trained either in Britain or in Argentina. The areas in which the Kleinians were pioneers -psychoanalysis of children and of psychotics - were the first aspects dealt with by this group in Brazil. Between 1950 and 1970 very dogmatic approaches were taken toward Kleinian ideas. As of 1980, with the publication of new translations of Melanie Klein's work and with the introduction of contemporary Kleinian thought, a more balanced use of this theoretical-technical model could been seen.

    Stepwise stress testing of different CAD-CAM lithium disilicate veneer application methods applied to lithium disilicate substructures

    Statement of problem: Whether a computer-aided design and computer-aided manufacture (CAD-CAM) fabricated high-translucency lithium disilicate veneer on a lithium disilicate substructure would increase the strength of the restoration compared with a traditional feldspathic porcelain veneer is unclear. Purpose: The purpose of this in vitro study was to evaluate the effect of different lithium disilicate veneer application methods on a lithium disilicate substructure on their biaxial flexural stress (BFS). Material and methods: Lithium disilicate disks were fabricated so that when combined with the veneering disks, they had a dimension of 12×1.2 mm. Experimental groups were as follows (n=15): resin-bonded lithium disilicate veneer, lithium disilicate veneer adhesively cemented to lithium disilicate; sintered lithium disilicate veneer, lithium disilicate veneer sintered to lithium disilicate; sintered feldspathic veneer, feldspathic porcelain applied to lithium disilicate; and monolithic lithium disilicate, the control group. Weibull distribution survival analysis was used to compare the differences in the resistance to fracture after fatigue. The total number of cycles was analyzed by using 1-way ANOVA (α=.05). A finite element analysis (FEA) was also performed. The maximum principal stress (MPS) was used as the failure criterion. Results: The sintered feldspathic veneer group had significantly lower fatigue resistance than sintered lithium disilicate veneer or resin-bonded lithium disilicate veneer (P<.05). The resin-bonded lithium disilicate veneer group showed significantly more fractured fragments than the other groups. No statistical difference was observed in the number of cycles. The lithium disilicate veneered groups presented similar resistance to fatigue as the monolithic specimens of the same overall dimensions. Higher peaks of MPS were observed for groups monolithic lithium disilicate, sintered lithium disilicate veneer, and sintered feldspathic veneer than for resin-bonded lithium disilicate veneer. Conclusions: Veneering a lithium disilicate substructure with a lithium disilicate veneer, bonded or sintered, increased resistance to fatigue compared with a feldspathic porcelain veneer. The lithium disilicate veneer groups had similar fatigue resistance to that of the monolithic group.

    Problematizando a reforma psiquiátrica na atualidade: A saúde mental como campo da práxis

    Mental Health, in the form of the Psychiatric Reform, and the Anti-Asylum Movement do not ignore the production of knowledge about that field, mainly due to the consolidation of Public Health as a field of knowledge. The article explores some authors who consider Mental Health as a new field of knowledge, introducing a new paradigm in the perception of health - Disease and Care -; however, the goal is to introduce Psychosocial Care as a means to enforce the transdisciplinary and multiprofessional practices. The possibility is that mental health produces developments in Health, consolidating the public policies. In practice, the hospital-centered and drug-based model still predominates, and there are setbacks to be overcome by taking advantage of loopholes capable of breaking with what is instituted.

    Considerações sobre a Ampliação da Intensao da Psicanálise numa Unidade de Pronto Atendimento

    This article intends to talk about the praxis of the psychologist, practitioner by the Freud-Lacan’s psychoanalysis in the Urgency of an Emergency Unit Care (EUC), in an attempt to operate with the Clinic of Urgency device. The Clinical of the Urgency device in our theoretical-clinical and ethical-political proposition aims to hear the suffering subjects. We discuss his position in this field of action in particular and how the attempts enlarge de hearing to extend that to a different place, from the traditional analytical setting. This is a theoretical exercise, an instrument of psychoanalytic work, a resource of psychoanalytic texts, authors, who use Freud-lacanian psychoanalysis to (re) think their analytical praxis. Therefore, we intend to share the psychologist’s action-reflection experience in this specific field of Health Attention. By exposing fragments of some cases, to think on these within the scope of the expanded psychoanalysis. We conclude that in this way of understanding the Urgency, a solid floor is found in the horizons of Psychoanalysis, essential for listening to the subject of the unconscious in its impasses and in its processes of falling ill.

    O Estado, a política liberal e a biopolítica

    This essay discusses some liberal political actions and their relations with the state. Considering some of their propositions, we ask: what power relations does that social practice respond to? Under what circumstances is the liberal politics interested in the government of men? From a study of the practices called corporate philanthropy we can indicate some strategies and tactics of such politics. Among which: the construction of the modern state, the development of various techniques of social control and the focus changes regarding the analysis of capitalism productive forces that intervene within the economic normality curves inherent in the population phenomena.

    Imigração e desamparo nos filhos de dekasseguis

    The phenomenon of the return of dekasseguis - migratory flow of Japaneses descendants to Japan - gained visibility in Brazil mainly after the global economic crisis of 2008, when the returns of Brazilians increased. The objective of this research was to investigate the migratory experience of these children of dekasseguis that accompanied his parents on their return to Brazil. Thus, we present a case study with a girl of thirteen, born in Japan and had recently arrived to Brazil with her parents. The results indicate that the childhood experienced in Japan produced deep identifications with Japanese culture; some go as far as not speaking Portuguese, considering themselves Japanese and founding themselves foreigners in Brazil. Feelings of estrangement and helplessness prevail in the experience of these dekassegui children.

    Filhos de decasséguis: desafios e dificuldades no retorno ao Brasil

    The Dekasegi phenomenon started in Brazil and Latin America in the 1980s and refers to Japanese descendants who migrated to Japan in search of employment. Recently, many families have returned to Brazil, along with their children. This research aimed to investigate the difficulties of those people in adapting or readapting to the Brazilian culture, particularly in the school environment. A clinical investigation method and interviews with parents and their children were used to that end. The findings show factors that contribute to worsen the difficulties of children in adapting to their new environment.

    Possibilidades nos Modos de Tratar a População em Situação de Rua *

    The number of people living in the streets of São Paulo and Brazil has been steadily increasing; a phenomenon that was before located in large urban centers, can be currently observed in several cities. This paper discusses the work in Social Assistance with homeless people, based on the praxis of the intercessor-worker, explaining the singularities of subjects and their territory. There is an absence of researches conducted by workers and a prevalence of papers that approach the life aspects of this population: who they are, how they live, and how they are seen. The objective was to critically analyze the reality in which the Social Assistance services are being implemented and propose ways of treating subjects, considering their subjectivity, so they get better situated in relation to the complex situation they live, and build access to social rights. In order to do so, the Intercessor Device (ID) was used, a modality of action that allows us to think and construct ways of acting and interceding in the reality of subjects and institutions, as well as produce knowledge and unconscious knowledge from this action. It is observed that the position of the worker in front of the demands and his theoretical and technical instruments, ID, make the difference in the analysis of the social context, in the approach of the complexity of the cases and in the construction of paths that allow subjects to access social rights. Although such rights are already guaranteed by law, it is verified that, in many cases, access to them must be effectively constructed, which demands intercessions between the subjects and the territorial services.

    Museu de Imagens do Inconsciente: considerações sobre sua história

    This article proposes to recover, partly, important facts in the history of the Museum of Images of the Unconscious, besides doing some notes about the repercussion of expositions organized by the Museum at the forty decade, more specifically in relation to the years of 1947 and 1949, that were significant in the Museum genesis process, besides relating two another events, that, as we think, are also of the highest importance at this matter.

    Diferença e igualdade nas relações de gênero: revisitando o debate

    This paper discusses the historical debate on difference and equality and on the changes undergone by gender relations under the impact of feminism, of the masculinity crisis and of the other economic, cultural and social changes currently underway. It is based on gender studies which seek to understand the processes of production of new forms of male and female subjectivities which are distant from the traditional gender stereotypes. The paper concludes that these changes point at the concrete possibility of building more democratic gender relations, an ideal which has been pursued since the beginning of the modern era for marriage and family, and whose cornerstones are the right to equality and the respect of difference.

    A instituição de saúde mental como dispositivo social de produção de subjetividade

    The institution of Mental Health is analyzed as a device for producing subjectivity. Though it deals with a unique production sector, the application of the analytical frame of reference of Social Formations and the market, as espoused by Marx, may bring to light some extremely useful aspects in the analysis of the composition and productive effects of such practices in the field of mental health, within our context, at this present moment in time. Two production modes stand out in clear contradiction: their arguments are consistent with either that of the capitalist mode of production or with the cooperative mode of production that preceded it; the definition of health will vary radically according to the case. It may be concluded that, within both the psychic and mental spheres, health production is inseparable from the production of subjectivity, being either syntonic or dystonic, according to the mode of production that provides the foundations for its fulfillment. With these elements, a new definition for psychic health is being proposed here as a basis for actions of Psychosocial Care.

    Social medicine in brazil: an alliance between sanitary education and popular pedagogy

    This study unfolds from the analysis of a set of documents collected during my doctoral research, which investigated the management of filth and its relationship with the governing of populations during the formation and development of modern cities. In this set, annals of Brazilian hygiene congresses, conducted during the first half of the twentieth century, a period characterized by the enlargement of techniques and practices of social medicine, were selected. Such annals of congresses and some laws of the time evidence the construction of an alliance between Sanitary Education and techniques of popular pedagogy, responsible for the population adherence to standards of conducts countersigned in social hygiene requirements. From Michel Foucault's writings on genealogical analysis, descent lines are described about healthy modes of conduct which, in turn, produce some effects in contemporary truth about personal and social hygiene.

    Trauma and testimony in ferenczi: An analysis of voices from tchernóbil by svetlana aleksiévith

    This article approaches the concepts of trauma and testimony as elaborated by Sándor Ferenczi and associates them to Svetlana Aleksiévitch’s book about the nuclear disaster of Chernobyl. It aims to show the value of Ferenczi’s observations on collective trauma by highlighting three topics: relational dissymmetry, process of denial and wounded narcissism. Aleksiévitch’s book allowed us to understand in what ways the author’s role resembles the one described by Ferenczi.

    Método Esther Bick: Observação da materialidade e dos fenômenos transicionais, durante o primeiro ano de vida

    O conceito de objeto transicional inaugurou, na psicanálise, a investigação do uso simbólico de objetos pelo bebê em suas primeiras experiências de separação. Outras facetas do papel da materialidade e de sua importância no desenvolvimento emocional e na clínica são exploradas em conceitos como objeto precursor, objeto autístico, objeto reconfortante, objeto acessório, objeto tutor, objeto testemunho e objeto de mediação. O objetivo do presente artigo é apresentar uma visão geral desses estudos e mostrar dados sobre a observação de um bebê prematuro pelo Método Esther Bick. A constituição, perda ou recuperação da experiência de transicionalidade – sob a perspectiva do desenvolvimento do uso dos objetos inanimados – indicam um papel marcadamente ativo do bebê na experiência de sustentar o jogo com sua mãe e, na ausência desta, em interagir com objetos tutores

    Objetos transicionais e o desenvolvimento da capacidade de incomodar

    Resumo: O objetivo do presente trabalho é sublinhar a relação histórica e conceitual entre a teoria dos fenômenos transicionais e a teoria da tendência antissocial, focalizando o uso pessoal e simbólico dos objetos inanimados. Winnicott (1956/2005) reconheceu que a deprivação, antes de ser o resultado de uma ruptura traumática do desenvolvimento emocional, é uma vivência corriqueira durante a maternagem e que o objeto transicional é indicador de que o bebê já alcançou, ou está em vias de alcançar, a capacidade de incomodar. Entretanto, quando traumática, a deprivação é seguida de um processo dissociativo em que o acesso à transicionalidade é, regressivamente, perdido e os objetos inanimados se tornam objetos impessoais. Uma reflexão decorrente desse tema é a do lugar dos objetos simbólicos e dos objetos impessoais em nosso cotidiano e seus efeitos no sofrimento psíquico da atualidade.     O Desenho da Figura Humana, com história (DFH-H) de uma criança sofrendo bullying, em contexto escolar

    El dibujo de la figura humana, con historia (DFH-H) es un procedimiento simple, barato, de fácil aplicación y que viene siendo utilizado en un servicio público de diagnostico y acompañamiento psicoterápico de niños y adolescentes, por estudiantes universitarios. Desde el punto de vista técnico, se trata de un estímulo de apercepción temática que se alinea a la tradición en Psicología de centralizar en la figura humana el proceso diagnóstico e interventivo y cuya ventaja es la universalidad de la propia representación de la figura humana. Básicamente, consiste en solicitar un único "dibujo de una persona", seguido de una historia sobre el diseño, la investigación y un título sobre el conjunto de la producción. El material necesario para la realización del procedimiento es una hoja de sulfito, lápiz negro apuntado y caucho. Nuestro objetivo es describir el procedimiento y la interpretación de los resultados enfocando los indicadores (1) cognitivo y (2) emocional y (3) proyectiva. Entre los otros aspectos proyectivos: los conflictos nodales, los sentimientos despertados por el diseño, aquello que es central, tamaño, formas distorsionadas, objetos, sombreados, rasgos, presencias de palabras, movimiento, transparencias. Se presentarán los resultados de la aplicación y análisis del DFH-H, en una entrevista inicial, realizada con un niño con queja de dificultades escolares. El DFH-H se mostró sensible a la evaluación inicial del caso y los encaminamientos.
