Avaliação das mudanças esqueléticas, dentárias e de tecidos moles criadas após terapia com aparelho de Herbst, em pacientes com má oclusão de Classe II divisão 1 em diferentes estágios de crescimento puberal

Abstract

Esse estudo retrospectivo, dividido em três publicações diferentes, objetivou comparar através de modelos ortodônticos 3D e radiografias em norma lateral, o resultado das mudanças dentárias, esqueléticas e de tecidos moles geradas após terapia com aparelho funcional fixo de Herbst, na correção da má oclusão de Classe II divisão 1. O estudo comparativo foi desenvolvido em 95 pacientes com crescimento craniofacial ativo. Foram alocados individualmente em 3 grupos, segundo o período de maturação das vértebras cervicais ao início do tratamento: antes, durante e pós surto de crescimento puberal. O material utilizado foi obtido dos arquivos do departamento da Clínica Infantil da Universidade estadual paulista “Júlio de Mesquita Filho” e de uma clínica ortodôntica particular (Aidar Ortodontia). A documentação de cada indivíduo foi escaneada e analisada de maneira qualitativa e quantitativa através de softwares computacionais. Os objetivos desse trabalho foram: (1) Comparar as mudanças cefalométricas em estruturas esqueléticas, dentárias e (2) de tecidos moles, assim como (3) a eficácia na correção da relação de molares e de caninos, em diferentes etapas do crescimento pubertário. Os resultados gerados foram organizados e submetidos a análise estatística, utilizando diferentes testes seguindo a natureza dos dados, sejam estes qualitativos ou quantitativos. O trabalho permitiu identificar algumas diferenças na resposta ao tratamento da má oclusão de Classe II com aparelho de Herbst nas diferentes etapas do desenvolvimento craniofacial.

This retrospective study, divided into three different publications, aimed to compare, through 3D orthodontic models and lateral x-rays, the result of dental, skeletal, and soft tissue changes generated after therapy with Herbst's' fixed functional appliance, in the correction of malocclusion of class II division 1. The comparative study was carried out in 95 patients with active craniofacial growth. They were allocated individually in 3 groups, according to the cervical vertebrae's maturation period at the beginning of treatment: before, during, and after a pubertal growth spurt. The used material was obtained from the archives of the Children's Clinic department of the São Paulo State University "Júlio de Mesquita Filho" and from a private orthodontic clinic (Aidar Orthodontics). The documentation of each individual was scanned and analyzed qualitatively and quantitatively through computer software. The objectives of this work were: (1) To compare cephalometric changes in skeletal, dental, and (2) soft tissue structures, as well as (3) the effectiveness in correcting the molar and canine relationship in different stages of pubertal growth. The results generated were organized and submitted to statistical analysis, using different tests according to the data's nature, whether qualitative or quantitative. The work made it possible to identify some differences in response to the treatment of Class II malocclusion with Herbst apparatus in the different stages of craniofacial development.
