Aplicação do fluxo digital na reabilitação anterior com implante imediato

Abstract

O objetivo deste estudo foi explanar um caso de reabilitação anterior planejado com fluxo digital. Paciente do sexo feminino de 58 anos de idade, ASA II, foi encaminhada devido à presença de reabsorção radicular externa no elemento 23, com histórico de sintomatologia e fístula. Ao exame físico a paciente reportava sintomatologia à percussão horizontal, ausência de fístula ou profundidade de sondagem. Na tomografia computadorizada por feixe cônico (TCFC) confirmou-se o diagnóstico de reabsorção radicular, sendo indicada a exodontia. Devido ao risco estético, com a presença de linha de sorriso média e fenótipo gengival fino optou-se pelo planejamento digital com instalação guiada e provisionalização imediata. Para o planejamento e confecção da guia foram utilizados os arquivos DICOM (Digital Imaging and Comunications in Medicine) da TCFC, e os arquivos STL obtidos no escaneamento intra-bucal. Os arquivos foram importados para o software CodiagnostiX no qual foi realizado o planejamento (CAD - computer-aided design), para então ser exportado e a guia obtida por manufatura aditiva (CAM-computer aided manufacturing). Foi instalado implante Titamax Ex CM Acqua 3,5x13mm, Neodent sem a abertura de retalho, preenchimento do gap vestibular com matriz inorgânica óssea bovina (Straumann Cerabone) e enxerto de tecido conjuntivo subepitelial. Uma vez obtida estabilidade primária adequada realizou-se a provisionalização imediata com a própria coroa do dente extraído. Após 6 meses foi confeccionada coroa em dissilicato de lítio cimentada sobre o abutment (Emax press, Ivoclar). O acompanhamento de 3 anos mostra saúde e estabilidade tecidual periimplantar e satisfação estética e funcional pela paciente.

The objective of this study was to explain a case of anterior dentoalveolar rehabilitation planned with digital flow. A 58-year-old female patient, ASA II, was referred due to the presence of external root resorption in tooth 23, with a history of symptoms and fistula. On physical examination, the patient reported symptoms of horizontal percussion, absence of fistula or probing depth. Cone beam computed tomography (CBCT) confirmed the diagnosis of root resorption, and exodontia was indicated. Due to the aesthetic risk, with the presence of a medium smile line and thin gingival phenotype, we opted for digital planning with guided installation and immediate provisionalization. To plan and create the guide, the DICOM (Digital Imaging and Communications in Medicine) files from TCFC were used, and the STL files obtained from intra-oral scanning. The files were imported into the CodiagnostiX software in which the planning was carried out (CAD - computer-aided design), to then be exported and the guide obtained by additive manufacturing (CAM-computer aided manufacturing). A Titamax Ex CM Acqua 3.5x13mm, Neodent implant was installed without opening a flap, filling the vestibular gap with inorganic bovine bone matrix (Straumann Cerabone) and subepithelial connective tissue graft. Once adequate primary stability was obtained, immediate provisionalization was carried out with the crown of the extracted tooth. After 6 months, a lithium disilicate crown was made and cemented onto the abutment (Emax press, Ivoclar). The 3-year follow-up shows health and peri-implant tissue stability and aesthetic and functional satisfaction for the patient.
