Abstract

Abstract (portuguese)

O objetivo deste estudo foi desenvolver e avaliar o efeito de um hidrogel termossensível poli (N-vinilcaprolactama) (PNVCL) com liberação controlada de tetraciclina e rosuvastatina na prevenção da osteonecrose dos maxilares relacionada a medicamentos (MRONJ). Para tanto, foi realizado inicialmente o desenvolvimento e a caracterização in vitro do hidrogel termossensível e incorporação das medicações. A caracterização contou com Espectroscopia de infravermelho por transformada de Fourier (FTIR), análise do grau de intumescência, degradação e liberação das medicações, isto é, rosuvastatina e tetraciclina. Além disso, um ensaio microbiológico foi realizado com saliva de pacientes com MRONJ, considerando crescimento do biofilme em aerobiose, anaerobiose e em meio para fungos. Em seguida, 30 ratas Wistar com 12 meses foram submetidas a ovariectomia bilateral. Após 2 meses, foi realizado, no dia -1 a instalação de uma ligadura de algodão ao redor do primeiro molar inferior esquerdo. No dia 0, foi iniciada a aplicação de Zoledronato através do protocolo 100μg/kg realizada a cada 3 dias, por 7 semanas. Após 3 semanas, foi realizada a exodontia do primeiro molar inferior esquerdo de todos os animais. Nesse momento, o alvéolo foi preenchido de acordo com o grupo ao qual pertencia. Para o grupo BC, nenhum material foi utilizado para preencher o alvéolo, somente o preenchimento com coágulo sanguíneo (controle negativo). No grupo CHX foi preenchido utilizado o gel de clorexidina 0,2%. Para o grupo PNVCL, foi preenchido com hidrogel termossensível de poli (N-vinilcaprolactama). Enquanto, para o grupo PNVCL + TC, preenchido com hidrogel PNVCL com tetraciclina em concentração de 25mg. Para o grupo PNVCL + RS o alvéolo foi preenchido com hidrogel de PNVCL com rosuvastatina. Já o grupo PNVCL+TC+RS teve o alvéolo preenchido com hidrogel de PNVCL com tetraciclina e rosuvastatina. Aos 28 dias, todos os animais foram anestesiados e realizadas fotos clínicas da região de exodontia. Em seguida, foi realizada a eutanásia e a dissecação da mucosa na região do primeiro molar para análise histológica, além da coleta das hemimandíbulas e seu encaminhamento para microtomografia computadorizada e análise histomorfométrica. Todos os dados quantitativos obtidos foram submetidos a análise estatística considerando p< 0.05. As análises de caracterização demonstraram grau de intumescência e degradação semelhantes para os grupos com hidrogel, com o grupo CHX apresentando valores superiores. A liberação de tetraciclina em 144 horas foi gradual, enquanto a rosuvastatina foi completamente liberada em 1 hora. O ensaio microbiológico demonstrou eficácia antimicrobiana de CHX, inclusive contra fungos, enquanto PNVCL+TC e PNVCL+TC+RS também apresentaram bom efeito antibacteriano. A análise clínica demonstrou tecido gengival mais satisfatório para PNVCL+TC+RS, com total recobrimento do tecido ósseo na área de exodontia, o que foi corroborado pela análise histológica da mucosa ao redor do local de extração, que demonstrou epitélio mais espesso para este grupo. Através da microtomografia e da análise histomorfométrica foi evidenciada maior formação de tecido ósseo para PNVCL+TC+RS, seguido de PNVCL+TC e PNVCL+RS. Tais resultados sugerem importante eficácia do hidrogel com tetraciclina e rosuvastatina, especialmente quando combinados, na modulação preventiva de MRONJ em ratas medicadas com Zoledronato.

Abstract (english)

The objective of this study was to develop and evaluate the effect of a thermosensitive poly (N-vinylcaprolactam) (PNVCL) hydrogel with controlled release of tetracycline and rosuvastatin in the prevention and control of medication-related osteonecrosis of the jaws (MRONJ). To this end, the development and in vitro characterization of the thermosensitive hydrogel were initially performed, along with the incorporation of the medications. The characterization included Fourier-transform infrared spectroscopy (FTIR), and the analysis of the swelling degree, degradation, and release of the medications, namely rosuvastatin and tetracycline. Additionally, a microbiological assay was performed with saliva from MRONJ patients to assess biofilm growth in aerobiosis, anaerobiosis, and fungal media. Subsequently, 30 twelve-month-old Wistar rats were subjected to bilateral ovariectomy. Two months later, on day -1, a cotton ligature was placed around the lower left first molar. On day 0, the administration of zoledronate was initiated using a protocol of 100 μg/kg every 3 days for 7 weeks. After 3 weeks, extraction of the lower left first molar was performed in all animals. At this point, the alveolar socket was filled according to the group to which the animal belonged. For the BC group, no material was used to fill the socket, allowing only filling with a blood clot (negative control). For the CHX group, the socket was filled with 0.2% chlorhexidine gel. For the PNVCL group, the alveolus was filled with a thermosensitive poly(N-vinylcaprolactam) hydrogel. The PNVCL + TC group was filled with PNVCL hydrogel containing tetracycline at a concentration of 25 mg. For the PNVCL + RS group, the alveolar socket was filled with PNVCL hydrogel containing rosuvastatin. In the PNVCL + TC + RS group, the alveolar socket was filled with PNVCL hydrogel containing tetracycline and rosuvastatin. At 28 days, all animals were anesthetized and clinical photographs of the extraction site were taken. Subsequently, euthanasia was performed, and the mucosa in the first molar region was dissected for histological analysis. In addition, the hemimandibles were collected and sent for micro-computed tomography and histomorphometric analysis. All quantitative data obtained were subjected to statistical analysis, considering the significance level of p < 0.05. The characterization analyses demonstrated similar degrees of swelling and degradation for the hidrogel groups, whereas the CHX group exhibited higher values. The release of tetracycline over 144 hours was gradual, whereas rosuvastatin was completely released within 1 hour. The microbiological assay revealed the antimicrobial efficacy of CHX, including antifungal activity, while PNVCL+TC and PNVCL+TC+RS also showed a good antibacterial effect. Clinical evaluation revealed a more satisfactory gingival tissue for the PNVCL+TC+RS group, with complete coverage of the bone tissue at the extraction site. This finding was corroborated by the gingival assessment, which demonstrated a thicker epithelium for this group. Microtomographic and histomorphometric analyses revealed greater bone tissue formation in the PNVCL+TC+RS group, followed by the PNVCL+TC and PNVCL+RS groups. These findings suggest a significant efficacy of the hydrogel containing tetracycline and rosuvastatin, particularly when combined, in the preventive modulation of MRONJ in zoledronate-treated rats.
