Perfil do impacto de saúde bucal e bem-estar de crianças pré-escolares

Abstract

Objetivos: i. estimar as propriedades psicométricas do Childhood Oral Health Impact Scale (ECOHIS) e do Quality of Life Evaluation Scale (AUQEI) quando aplicados à crianças pré-escolares; ii. estimar o bem-estar e o perfil do impacto de saúde bucal das crianças; iii. verificar o impacto dos indicadores clínicos no perfil de saúde bucal das crianças. Métodos: Participaram do estudo mães e crianças em idade pré-escolar matriculadas em instituições municipais de ensino público do município de Araraquara-SP. As propriedades psicométricas dos instrumentos aplicados à amostra foram analisadas a partir de análise fatorial confirmatória. As validades convergente e discriminante e a confiabilidade foram investigadas. Utilizou-se análise multigrupos para avaliação da invariância fatorial de cada instrumento em amostras independentes. Após ajustamento dos modelos fatoriais dos instrumentos, os escores médios dos fatores dos instrumentos foram calculados e comparados entre subgrupos delimitados pelas características amostrais por meio de análise de variância (ANOVA). Resultados: O modelo fatorial original do ECOHIS apresentou adequado ajustamento aos dados. Atestou-se invariância forte dos instrumentos em amostras independentes. Os indivíduos que apresentaram experiência de cárie e com menor renda familiar apresentaram maior impacto da saúde bucal em suas vidas. O modelo fatorial original do AUQEI não convergiu, apresentou colinearidade entre os fatores e alta correlação entre os itens 2 e 3. Foi sugerido um modelo unifatorial com exclusão de um item que apresentou adequado ajustamento aos dados. Não houve diferença estatística nos escores de bem-estar subjetivo de acordo com o sexo da criança e idade, estado civil, exercício de atividade laboral e estrato econômico das mães e 67,5% das crianças apresentaram percepção de bem-estar positivo. Conclusão: Os dados coletados com o ECOHIS e o AUQEI junto a crianças pré-escolares foram válidos e confiáveis. A maioria das crianças apresentou bem-estar subjetivo positivo sem relação com as características demográficas da criança ou da mãe. As variáveis estrato econômico, experiência de cárie e nível de placa apresentaram influência significativa na percepção do impacto da saúde bucal na vida das crianças.

Objetives: i. to estimate the psychometric properties of the Childhood Oral Health Impact Scale (ECOHIS) and the Quality of Life Evaluation Scale (AUQEI) when applied to pre-school children; ii. to estimate the well-being and profile impact of children’s dental health; iii. verify the impact of clinical indicators on the profile of dental health of children. Methods: Mothers and children enrolled in the municipal pre-schools of the Brazilian public education system in the city of Araraquara, in the State of São Paulo, took place in this study. The psychometric properties of the tools applied to the sample were analysed using confirmatory factorial analysis. Convergent and discriminant validities, as well as reliability of the model were assessed using multi-group analysis for estimating factorial invariance of each tool item in independent samples. After adjustment of the factorial models, the average scores of the factors were calculated and compared between the sub-groups defined by the sample characteristics through variance analysis (ANOVA). Results: The original ECOHIS model showed satisfactory adjustment to the data. The individuals who had experience of caries and lower household income had a higher impact of dental health on their lives. The original AUQEI factorial model did not converge, presenting collinearity between the factors and high correlation between items 2 and 3. A unifactorial model was then suggested, with the exclusion of one item which was adequate for adjusting the data. No significant statistical difference was observed in the scores of subjective well-being according to the child’s gender and age, as well as to the marital status, work sector and economic stratum of the mothers, with 67.5% of the children showing positive well-being. Conclusion: The data collected with the AUQEI and ECOHIS from pre-school children were valid and reliable. Most children analysed showed positive subjective well-being, showing no relationship with demographic parameters of the child or mother. The variables economic stratum, experience of caries and plaque rate had a significant influence on the perception of dental health on the children’s lives.
