Efeitos biológicos do extrato de erva-mate (Ilex paraguariensis A. St.-Hil.) sobre a periodontite apical em ratos Wistar diabéticos

O objetivo foi avaliar o potencial biológico do extrato de erva-mate (Ilex paraguariensis, EM) associado à periodontite apical (PA) em ratos Wistar diabéticos (D) mimetizando o uso habitual da erva. Foram utilizados 64 ratos Wistar machos, divididos em 8 grupos: Grupo controle (CO), tratado com EM (EM); PA; PA+EM; D; D+EM; D+PA; e D+PA+EM. O diabetes mellitus (DM) foi induzido no dia 0 com estreptozotocina (35 mg/kg) e, 7 dias após, iniciou-se a administração oral de EM (80 mg/kg/dia). A PA (dia 35) foi induzida por quatro exposições pulpares. Na eutanásia (dia 65), amostras sanguíneas foram coletadas para hemograma, IL-6, TNF-α, IL-1β e IL-10 (ELISA), capacidade antioxidante total (CAT) e análise de substâncias reativas ao ácido tiobarbitúrico (TBARS). As hemimandíbulas direitas foram analisadas histologicamente quanto ao infiltrado inflamatório e à extensão das lesões, e as hemimaxilas direitas quanto ao volume ósseo periapical (microtomografia computadorizada). Durante o experimento, foram monitorados o peso corporal e a glicemia. Os resultados foram submetidos a testes estatísticos específicos para cada caso (5%). Os animais normoglicêmicos apresentaram ganho de peso progressivo, enquanto os D mantiveram o peso estável (p<0.0001). A hiperglicemia confirmou a indução do DM (p<0,0001). As lesões periapicais foram agravadas pela presença do DM, com maior infiltrado, maior extensão da lesão e maior perda de volume ósseo no grupo D+PA (p<0,05). A associação da EM nos grupos PA+EM e D+PA+EM, resultou em leões menos severas comparado aos PA e D+PA, respectivamente (p<0,05). O número de plaquetas aumentou no grupo D+PA em relação aos grupos normoglicêmicos (p<0,05). Os leucócitos e os linfócitos foram maiores no grupo PA e no PA+EM, em comparação aos CO e EM (p<0,05). Porém, o grupo PA+EM não diferiu estatisticamente do grupo PA nem do CO (p>0,05). O grupo D+PA apresentou o maior aumento de leucócitos, linfócitos e neutrófilos entre todos os grupos (p<0,05). Porém, a associação D+PA+EM controlou o aumento dos os três tipos celulares (p<0,05). A presença da PA aumentou os níveis séricos de IL-6, TNF-α e IL-1β e diminuiu os de IL-10 (p<0,05), com efeito mais pronunciado nos animais diabéticos (p<0,05). A EM foi capaz de controlar a expressão de IL-6 nos grupos PA+EM (p<0,0001) e D+PA+EM (p=0,0022), e a de TNF-α no grupo D+PA+EM (p=0,0279). O TBARS foi aumentado na presença do DM (p<0,0001) e da PA (p<0,0001), com maior valor no grupo D+PA (p<0,05). O TBARS diminuiu nos grupos PA+EM e D+PA+EM, em relação aos PA e D+PA, respectivamente (p<0,05). A CAT foi menor na presença do DM (p<0,001) e da PA (p<0,001). Conclui-se que a EM foi capaz de atenuar os efeitos deletérios ocasionados pela associação entre PA e DM, controlando a expressão sérica de IL6 e TNF-α, os valores de leucócitos, neutrófilos e linfócitos, bem como o estresse oxidativo, por meio da redução da peroxidação lipídica. Adicionalmente, foi associado a um menor grau de severidade da PA, com menor infiltrado inflamatório e menos reabsorção óssea.

The aim was to evaluate the biological properties of yerba mate (Ilex paraguariensis, YM) associated with apical periodontitis (AP) in diabetic (D) Wistar rats, mimicking the habitual use of the herb. Sixty-four male Wistar rats were used and divided into eight groups: Control (CO), YM-treated (YM), AP, AP+YM, D, D+YM, D+AP, and D+AP+YM. Diabetes mellitus (DM) was induced (day 0) by streptozotocin (35 mg/kg), and YM oral administration (80 mg/kg/day) began 7 days later. AP was induced (day 35) by four pulp exposures. At euthanasia (day 65), blood samples were collected for blood count, IL-6, TNF-α, IL-1β, and IL-10 (ELISA), total antioxidant capacity (TAC), and thiobarbituric acid reactive substances (TBARS). The right hemimandibles were histologically analyzed for inflammatory infiltrate and lesion extension, while the right hemimaxillae were evaluated for periapical bone loss volume by micro-computed tomography. Body weight and blood glucose levels were monitored throughout the experiment. Data were analyzed using specific statistical tests for each dataset (5% significance level). Normoglycemic animals showed progressive body weight gain, whereas D animals maintained stable body weight (p<0.0001). Hyperglycemia confirmed DM induction (p<0.0001). Periapical lesions were aggravated by DM, with greater inflammatory infiltrate, lesion extension, and bone loss volume in the D+AP group (p<0.05). The association of YM in the AP+YM and D+AP+YM groups resulted in less severe lesions compared to AP and D+AP, respectively (p<0.05). Platelet count increased in the D+AP group compared to the normoglycemic groups (p<0.05). Leukocyte and lymphocyte counts were higher in the AP and AP+YM groups compared with the CO and YM groups (p<0.05). However, the AP+YM group did not differ significantly from either the AP or CO groups (p>0.05). The D+AP group exhibited the greatest increase in leukocyte, lymphocyte, and neutrophil counts among all groups (p<0.05). However, the D+AP+YM association attenuated the increase in all three cell types (p<0.05). The presence of AP increased serum levels of IL-6, TNF-α, and IL-1β, while reducing IL-10 levels (p<0.05), with a more pronounced effect in diabetic animals (p<0.05). YM was able to modulate IL-6 expression in the AP+YM (p<0.0001) and D+AP+YM (p=0.0022) groups, as well as TNF-α expression in the D+AP+YM group (p=0.0279). TBARS levels were increased in the presence of DM (p<0.0001) and AP (p<0.0001), with the highest values observed in the D+AP group (p<0.05). TBARS levels were reduced in the AP+YM and D+AP+YM groups compared with the AP and D+AP groups, respectively (p<0.05). Catalase (CAT) activity was lower in the presence of DM (p<0.001) and AP (p<0.001). In conclusion, YM attenuated the deleterious effects associated with the combination of AP and DM by modulating serum IL-6 and TNF-α expression, reducing leukocyte, neutrophil, and lymphocyte counts, and mitigating oxidative stress through decreased lipid peroxidation. Additionally, YM was associated with a lower severity of AP, characterized by reduced inflammatory infiltrate and decreased bone resorption.
