Acurácia da avaliação radiográfica em comparação com a medição tomográfica do grau de torção da tuberosidade tibial em cães com luxação patelar medial

Abstract

A torção tibial descreve a mudança na orientação entre o eixo articular proximal e o distal no plano transversal da tíbia. O objetivo desse estudo foi avaliar a acurácia do método radiográfico na avaliação da torção tibial em cães com luxação patelar medial (LPM), tendo como referência as mensurações tomográficas. Foram selecionadas retrospectivamente imagens radiográficas e tomográficas de 40 tíbias de cães que receberam o diagnóstico de LPM. O ângulo de torção tibial foi calculado por três avaliadores utilizando o método de mensuração radiográfico e por quatro avaliadores utilizando o método de mensuração tomográfico. O coeficiente de correlação intraclasse (CCI) interobservador foi calculado para cada método, as médias de cada método foram comparadas estatisticamente entre si e foram feitas comparações entre os resultados das mensurações radiográficas e tomográficas. Os resultados evidenciaram que as mensurações tomográficas apresentaram alta concordância (CCI= 0,99) entre os avaliadores e as medições radiográficas apresentaram moderadas concordância (CCI=0,63) entre os avaliadores. Não houve diferença significativa (p=0,122) entre as médias das avaliações tomográficas e radiográficas. Noventa e dois vírgula cinco porcento (37/40) das mensurações tomográficas e 25% (10/40) das medições radiográficas tiveram até cinco graus de diferença entre os avaliadores. A técnica de mensuração radiográfica da torção tibial em cães com LPM é reprodutível e passível de aplicação clínica quando a tomografia computadorizada não estiver disponível para essa avaliação.

Tibial torsion describes the angular relationship of the proximal articular axis relative to the distal articular axis of the tibia about its longitudinal axis. The aim of this study was to evaluate the accuracy of the radiographic method in evaluating tibial torsion in dogs with medial patellar luxation (MPL), using tomographic measurements as a reference. Radiographic and tomographic images of 40 tibias from dogs diagnosed with MPL were retrospectively selected. The tibial torsion angle was calculated by three observers using the radiographic measurement method and by four observers using the tomographic measurement method. The inter-observer intraclass correlation coefficient (ICC) was calculated for each method, the means of each method were statistically compared with each other, and comparisons were made between the results of radiographic and tomographic measurements. The results showed that tomographic measurements had high agreement (ICC=0.99) between observers and radiographic measurements had moderate agreement (ICC=0.63) between observers. There was no significant difference (p=0.122) between the means of tomographic and radiographic evaluations. Ninety-two-point five percent (37/40) of the tomographic measurements and 25% (10/40) of the radiographic measurements had up to five degrees of difference between the evaluators. The technique for radiographic measurement of tibial torsion in dogs with MPL is reproducible and capable of clinical application when computed tomography is not available for this evaluation.
