Tratamento de fratura bilateral de mandíbula: relato de caso

Abstract

A prática da cirurgia maxilofacial pode ser muito desafiadora e necessita análise criteriosa de todas as variáveis envolvidas dentro de um caso, para que se possa chegar a um correto diagnóstico e escolha adequada de tratamento. Este trabalho tem como objetivo relatar um caso de um paciente do sexo feminino, 21 anos de idade, vítima de um acidente automobilístico atendida pela equipe de Cirurgia e Traumatologia Buco-Maxilo-Facial da Santa Casa de Araçatuba. Paciente foi atendida em pronto-socorro, sem queixas álgicas espontâneas, entretanto queixava-se durante a manipulação da mandíbula. Durante o exame clínico, apresentava a abertura bucal mantida e os côndilos palpáveis. Ao exame intraoral foi observada a presença de um coágulo em fundo de vestíbulo mandibular, crepitação óssea em parassínfise direita, além de edema em face. Através do exame de tomografia computadorizada da face, verificou-se a presença de fraturas mandibulares (ângulo mandibular esquerdo e parassínfise direita). Diante dos sinais clínicos e evidências radiográficas, optou-se por realizar a redução e fixação das fraturas com uso de placas e parafusos do sistema 2.0 por meio de acesso intraoral e técnica de Champy na região de ângulo. No pós operatório foi verificado reestabelecimento da oclusão funcional e anatômica, sem queixas ou danos aos nervos ou teciduais exacerbados. Na proservação de 6 meses observou-se completa cicatrização da região operada e reparo satisfatório da área antes fraturada. O tratamento realizado foi eficaz para reestabelecimento anatômico e funcional do paciente com menor comorbidade associada.

The practice of maxillofacial surgery can be very challenging and requires careful analysis of all the variables involved within a case, so that a correct diagnosis and appropriate choice of treatment can be reached. This work aims to report a case of a female patient, 21 years old, victim of a car accident treated by the Oral and Maxillofacial Surgery and Traumatology team at Santa Casa de Araçatuba. The patient was treated in the emergency room, without spontaneous pain complaints, however, she complained during manipulation of the jaw. During the clinical examination, her mouth opening was maintained and her condyles were palpable. During intraoral examination, the presence of a clot was observed at the bottom of the mandibular vestibule, bone crepitation in the right parasymphysis, in addition to edema on the face. Through a computed tomography examination of the face, the presence of mandibular fractures was verified (left mandibular angle and the right parasymphysis). Given the clinical signs and radiographic evidence, it was decided to reduce and fix the fractures using plates and screws from the 2.0 system through intraoral access and the Champy technique in the angle region. Post-operatively, functional and anatomical occlusion was reestablished without complaints or exacerbated nerve or tissue damage. During the 6-month follow-up, complete healing of the operated region and satisfactory repair of the previously fractured area were observed. The treatment carried out was effective in restoring the patient’s anatomical and functional status with less associated comorbidity.
