Radiation-induced pleomorphic sarcoma of the maxillary sinus

Abstract

Sarcoma pleomórfico indiferenciado (SPI) é uma neoplasia maligna de alto grau que raramente se desenvolve na região da cabeça e pescoço. No entanto, pode ser observada em pacientes submetidos à radioterapia prévia. Relatamos o caso de uma mulher de 56 anos que desenvolveu um SPI do seio maxilar após ser submetida a radioterapia para tratamento de um Linfoma Difuso de Grandes Células B (LDGCB) extranodal em seio esfenoidal com dose acumulada de 45 Grays. Após o tratamento, a paciente obteve remissão completa do linfoma e continuou a ser monitorada. Cinco anos após o tratamento do LDGCB, ela apresentou uma lesão ulcerada em gengiva superior. Segundo ela, a lesão tinha um mês de evolução. O exame microscópico revelou um tumor maligno de alto grau com células pleomórficas. As células tumorais foram negativas para S-100, CD45, CD30, citoqueratina, CD20, CD34 e desmina, e positivas para p63, actina de músculo liso e Ki-67. A tomografia computadorizada mostrou uma imagem hipodensa no seio maxilar esquerdo. Os achados levaram ao diagnóstico de um SPI do seio maxilar. A paciente faleceu antes de iniciar o tratamento paliativo devido à rápida progressão do SPI. Este relato destaca a importância de se considerar o SPI no diagnóstico diferencial de lesões de crescimento rápido em pacientes previamente tratados com radioterapia na região de cabeça e pescoço.

Undifferentiated pleomorphic sarcoma (UPS) is a high-grade malignancy that rarely develops in the head and neck region. However, it can be observed in patients who have undergone prior radiotherapy. Herein, we report the case of a 56-year-old woman who developed an UPS of the maxillary sinus after undergoing radiotherapy for extranodal Diffuse Large B-Cell Lymphoma (DLBCL) of the sphenoid sinus with an accumulated dose of 45 Grays. After treatment, the patient achieved complete remission from lymphoma and continued to be monitored. Five years after DLBCL treatment she showed an ulcerated lesion on the upper gingiva. According to her, the lesion had one month of evolution. Microscopic examination revealed a high-grade malignant tumor with pleomorphic cells. Tumor cells were negative for S-100, CD45, CD30, cytokeratin, CD20, CD34, and desmin, and positive for p63, smooth muscle actin, and Ki-67. Computed tomography showed hypodense image in the left maxillary sinus. The findings led to the diagnosis of UPS of the maxillary sinus. The patient died before starting palliative treatment owing to the rapid UPS progression. This report highlights the importance of considering UPS in the differential diagnosis of rapidly growing lesions in patients previously treated with radiotherapy in head and neck region.
