Resumo

A endometrite persistente pós-cobertura (EPPC) é uma afecção que acomete uma parcela significativa das éguas que estão em programas de reprodução. Em vista disso, o objetivo desta revisão de literatura foi abordar a fisiopatologia da EPPC e seus tratamentos. Após a cobertura ou inseminação artificial (IA) é fisiológico que ocorra inflamação no interior do útero a fim de promover a limpeza uterina e drenagem dos debris celulares e contaminantes. Éguas que são susceptíveis a EPPC permanecem com acúmulo de líquido uterino e elevado infiltrado neutrofílico no interior do útero por mais de 36 horas após o contato com o sêmen. Para que essas fêmeas possam ser utilizadas na reprodução e se obtenha índices de fertilidade aceitáveis é necessário controlar a inflamação, oferecendo um ambiente uterino adequando para o embrião. A maioria dos tratamentos usados para as éguas com EPPC são paliativos, como a utilização de fármacos ecbólicos e lavagem uterina para auxiliar na retirada do líquido e os polimorfonucleares (PMNs) acumulados no interior do útero. Outros tratamentos tem sido sugeridos, como os imunomoduladores que atuam bloqueando a resposta inflamatória inicial, evitando a liberação de mediadores inflamatórios. Recentemente tem sido mencionada a utilização de anti-inflamatórios não esteroidáis (AINEs) para o tratamento de éguas susceptíveis, que atuam bloqueando a cicloxigenase (COX) interrompendo assim liberação das prostaglandinas (PGs) que são as causadoras do processo inflamatório. Um dos AINEs utilizado em equinos para o tratamento de outras alterações é o firocoxib, por ser seletivo a COX-2, não atua sobre a COX-1, evitando os efeitos adversos por essa via fisiológica. Esse medicamento pode se tornar uma opção para o tratamento de EPPC após ser comprovada sua eficiência.
The persistente post-breedind endometritis (PPBE) is one pathology that affects significant part of the mares that are in breeding programs. The objective of this study was to make a literature review on the PMIE and their possible treatments. After mating or artificial insemination (AI) an physiological inflammation happens inside the uterus for uterine cleaning and drainage of cellular debris and contaminants. Mares that are susceptible to this type of endometritis can not make the proper uterine cleaning and remain with accumulated fluid and a high neutrophilic infiltrate inside the uterus for more than 36 hours after contact with semen. For that these females can be used for reproduction and be able to get acceptable fertility rates is necessary the control of this inflammation, offering a suitable uterine environment for the embryo. Most treatments that are used for PMIE mares are palliative, such as the use of ecbolic drugs and uterine wash to aid in removal of liquid and polymorphonuclear (PMNs) accumulated in the uterus. Other treatments have been suggested, such as the imunomodulatory treatment that act by blocking the initial inflammatory response, preventing the release of inflammatory mediators. Recently it has been mentioned the use of nonsteroidal anti-inflammatory drugs (NSAIDs) for the treatment of susceptible mares, working by blocking the cyclooxygenase (COX) and interrupting the release of prostaglandins (PGs) that are the cause of the inflammatory process. A NSAID used in horses for treatment of other pathologies is firocoxib, being a selective COX-2 drug that has no effect on COX-1, avoiding the adverse effects by this physiological route. This medicine may become an option for the treatment of PMIE after proven effectiveness tests.
