Resumo

Objetivo: O presente estudo foi realizado com o objetivo de verificar se as doenças sistêmicas e hábitos de fumar e ingerir bebidas alcoólicas estavam associadas à baixa visão e cegueira assim como a relação das doenças sistêmicas com as causas de baixa visão e cegueira em uma determinada população. Material e Método: Estudo transversal, observacional, de base populacional, no qual foram avaliadas 897 pessoas com idade igual ou superior a 18 anos, residentes no Município de Pratânia, São Paulo, nos anos de 2007 e 2008. Os sujeitos foram atendidos na unidade de saúde do município e na unidade móvel oftalmológica. Os exames permitiram detectar as principais afecções oculares causadoras de baixa visão ou cegueira, assim como as principais afecções sistêmicas. Os dados foram transferidos para Tabela Excel e analisados estatisticamente utilizando o programa SAS, versão 9.3. Estatísticas descritivas e análises de associação buscaram conhecer a relação entre baixa visão ou cegueira com as doenças sistêmicas e hábitos de fumar ou ingerir bebida alcoólica. Resultados: Dentre as pessoas avaliadas, foram encontrados 19 cegos (2,1%) e 43 (4,8%) indivíduos portadores de baixa visão levando-se em conta a acuidade visual (AV) verificada. Após correção óptica, diminuiu para 4 (0,5%) cegos e 15 (1,7%) com baixa visão. Houve associação significativa entre idade avançada com hipertensão arterial sistêmica (81,8% dos idosos eram hipertensos), diabetes mellitus (20,4% dos idosos eram diabéticos), dislipidemia (48,5% dos idosos eram dislipidêmicos), obesidade (42,4% dos idosos eram sobrepesos e 32,6%, obesos), relação cintura-quadril (83,3% dos idosos tinham relação aumentada) e ingestão diária de bebida alcoólica (19,4% dos idosos bebiam todos os dias). Ainda associação entre menor idade com tabagismo (24,1% dos adultos jovens eram fumantes). Quanto ao sexo, houve associação significativa entre sexo feminino e obesidade (34,1% das mulheres eram obesas). Assim também entre sexo masculino com tabagismo (31,1% dos homens eram fumantes) e consumo de álcool tanto na regularidade (45,9% dos homens ingeriam álcool regularmente) quanto na frequência semanal (16,9% dos homens ingeriam álcool todos os dias). Quanto à AV verificada, a associação significativa foi entre baixa visão e cegueira com idade avançada (p< 0,0001), diabetes (20,9% dos indivíduos de baixa visão e 5,3% dos cegos eram diabéticos, p= 0,025), obesidade (p= 0,027), catarata (p= 0,005) e uveíte (p= 0,040). Quanto à AV corrigida, a idade avançada (80% dos indivíduos com baixa visão e 100% dos cegos eram idosos) e a obesidade (p= 0,006) permaneciam em associação com baixa visão e cegueira juntamente à relação cintura-quadril (p= 0,020). Na análise multivariada, a idade avançada e a hipertensão arterial sistêmica estavam associadas ao desenvolvimento de baixa visão ou cegueira (p< 0,05) na AV verificada. A idade avançada e a uveíte na AV corrigida. A catarata apresentou associação significativa com hipertensão, diabetes, dislipidemia, obesidade, relação cintura-quadril e tabagismo (p< 0,05). Conclusão: Há necessidade de melhorar a assistência e prevenir doenças sistêmicas como hipertensão, diabetes, dislipidemia e obesidade, e os hábitos de fumar e ingerir bebidas alcoólicas a fim de reduzir as chances do indivíduo desenvolver baixa visão e cegueira.
Purpose: The present study was conducted with the objective of verifying if the systemic diseases and smoking habits and drinking alcoholic drinks were associated with low vision and blindness as well as the relation of systemic diseases with the causes of low vision and blindness in a given population. Material and Method: A cross-sectional study, observational, population-based study that evaluated 897 people aged 18 years or more in the municipality of Pratânia, São Paulo, in the years 2007 and 2008. The subjects were attended at the health unit of the municipality and the ophthalmological mobile unit. The tests revealed the main ocular disorders that cause low vision or blindness, as well as the main illnesses systemic. The data were transferred to the Excel table and statistically analyzed using SAS software, version 9.3. Descriptive statistics and analyzes of association sought to know the relationship between the low vision or blindness with the systemic diseases and smoking habits or drinking alcoholic drinks. Results: Among the people evaluated were found 19 (2.1%) blind and 43 (4.8%) individuals with low vision in the assessment of visual acuity checked. After optical correction, it decreased to 4 (0.5%) blind and 15 (1.7%) with low vision. There was a significant association between advanced age with systemic arterial hypertension (81.8% of the elderly were hypertensive), diabetes (20.4% of the elderly were diabetic), dyslipidemia (48.5% of the elderly were dyslipidemic), obesity (42.4% of the elderly were overweight and 32.6%, obese), waist-to-hip ratio (83.3% of the elderly showed increased ratio) and daily consumption of alcoholic drinks (19.4% of the elderly drank every day). There was also an association between young adults with smoking (24.1% of young adults were smokers). In relation to gender, there was a significant association between female gender and obesity (34.1% of the women were obese). This also occurred between male gender and smoking habits (31.1% of the men were smokers) and consumed alcohol both in relation to regularity (45.9% of the men consumed alcohol regularly) and weekly frequency (16.9% of the men drank alcohol every day). In relation to visual acuity checked, the significant association was between low vision and blindness with advanced age (p< 0.0001), diabetes (20.9% of the individuals with low vision and 5.3% of blind people were diabetic, p= 0.025), obesity (p= 0.027), cataracts (p= 0.005) and uveitis (p= 0.040). In relation to corrected VA, advanced age (80% of subjects with low vision and 100% of blind were elderly) and obesity (p= 0.006) remained in association with low vision and blindness together with waist-to-hip ratio (p= 0.020). In the multivariate analysis, advanced age and systemic arterial hypertension were associated with the development of low vision or blindness (p< 0.05) in the VA checked. Advanced age and uveitis in corrected VA. Cataracts showed significant association with hypertension, diabetes, dyslipidemia, obesity, waist-to-hip ratio and smoking habits (p< 0.05). Conclusion: It is necessary to improve assistance and avoid systemic disorders such as hypertension, diabetes, dyslipidemia and obesity, and smoking habits and consumption of alcoholic drinks in order to reduce the chances of the individual to develop low vision and blindness.
