Estabilidade de cor, translucidez e topografia de superfície de materiais cerâmicos monolíticos injetados e CAD/CAM caracterizados extrinsecamente após escovação e termociclagem

Resumo

Esse estudo avaliou, in vitro, propriedades ópticas e de superfície de cerâmicas monolíticas, caracterizadas extrinsecamente, à base de dissilicato de lítio (IPS e.max® CAD e Press) e zircônia totalmente estabilizada de alta translucidez (Prettau® 4 Anterior®) após escovação com diferentes dentifrícios e termociclagem. Espécimes (N=40) em formato de barra (12 x 10 x 1,5 mm) foram obtidos, cristalizados/sinterizados, polidos e divididos nos grupos: STAIN/GLAZE (n=20), no qual receberam aplicação de stain + glaze; ou GLAZE (n=20), recebendo somente glaze. Em seguida, foram novamente divididos e submetidos ao envelhecimento termomecânico, como segue: 1- Escovação Convencional (50.000, 100.000 e 150.000 ciclos; 1,96 N, 1,0 Hz - Colgate Total 12 Clean Mint) + Termociclagem (6.000, 12.000 e 18.000 ciclos; 5-55ºC, 30 s); ou 2- Escovação Whitening (50.000, 100.000 e 150.000 ciclos; 1,96 N, 1,0 Hz - Colgate Total 12 Professional Whitening) + Termociclagem (6.000, 12.000 e 18.000 ciclos; 5-55ºC, 30 s). O envelhecimento foi conduzido intercalando ciclos de escovação e termociclagem, com simulação clínica de 5, 10 e 15 anos. Dados, na condição inicial e após 5, 10 e 15 anos de envelhecimento, foram obtidos com espectrofotômetro (VITA Easyshade® V) para os cálculos da alteração de cor (ΔE) e translucidez (T), de acordo com parâmetros CIELAB (L*, a* e b*) e CIEDE2000 (L*, C* e h*). Imagens foram obtidas em microscopia confocal a laser 3D para avaliação da topografia de superfície das amostras (n=3) nos diferentes períodos. Os dados foram submetidos aos testes de normalidade (Shapiro-Wilk) e homogeneidade de variância (Levene). Foram conduzidas 2-way ANOVA de medidas repetidas mistas (α=.05), seguidas por teste LSD de Fisher, para a comparação aos pares nas condições experimentais para as variáveis quantitativas dependentes (ΔEab, ΔE00 e T). ANOVAs revelaram significância estatística para a interação Material x Período da Análise. Quando os materiais são comparados, Prettau 4 Anterior apresentou maior alteração de cor (ΔEab e ΔE00). Nenhum grupo experimental produziu valores de ΔEab e ΔE00 clinicamente perceptíveis. Exceto para grupos GLAZE escovados com dentifrício convencional, a translucidez dos materiais avaliados seguiu, em ordem decrescente, a desigualdade: e.max Press > e.max CAD > Prettau 4 Anterior. No geral, as imagens da topografia de superfície revelaram alterações superficiais, diretamente proporcionais ao aumento do período, em anos, do envelhecimento termomecânico, independente do grupo e dentifrício, sendo mais significativas e relevantes para zircônia Prettau 4 Anterior. A cor, translucidez e topografia de superfície de cerâmicas monolíticas injetadas e CAD/CAM, maquiadas e glazeadas ou somente glazeadas alteraram após a escovação com diferentes dentifrícios, seguida da termociclagem.
This in vitro study investigated optical and surface properties of extrinsically stained monolithic ceramics based on lithium disilicate (IPS e.max® CAD and Press) and high translucent fully-stabilized zirconia (Prettau® 4 Anterior®) after toothbrushing with conventional or whitening dentifrices and thermocycling. Bar-shaped specimens (n=40; 12 x 10 x 1.5 mm) were obtained, crystallized/sintered, polished, and divided in two groups: STAIN/GLAZE (n=20), in which the samples received stain and glaze; or GLAZE (n=20), glazed only. Each group were subdivided in other two groups to be subjected to the thermomechanical aging, as follow: 1- Conventional Toothbrushing (50,000; 100,000 and 150,000 cycles; 1.96 N; 1.0 Hz – Colgate Total 12 Clean Mint) + Thermocycling (6,000; 12,000 and 18,000 cycles; 5-55ºC, 30 s); or 2- Whitening Tootbrushing (50,000; 100,000 and 150,000 cycles; 1.96 N; 1.0 Hz – Colgate Total 12 Professional Whitening) + Thermocycling (6,000; 12,000 and 18,000 cycles; 5-55ºC; 30 s). Aging was performed alternating cycles of toothbrushing and thermocycling, to simulate 5, 10 and 15 years of clinical service. Data, in the initial condition and after 5, 10 and 15 years of aging were obtained with a spectrophotometer (VITA Easyshade® V) to calculate changes in color (ΔE) and translucency (T), according to the CIELAB (L*, a* and b*) and CIEDE2000 (L*, C* and H*) parameters. Images were obtained in 3D laser confocal microscopy to evaluate the surface topography of the samples (n=3) in the different periods. Data were submitted to normality (Shapiro-Wilk) and homogeneity of variance (Levene) tests. 2-way ANOVAs followed by Fisher LSD post-test were conducted for pairwise comparison under the different conditions for the quantitative variables (ΔEab, ΔE00 and T), with significance level of 95%. ANOVAs showed statistical significance for the Material x Period of Analysis interaction. When the different ceramic materials are compared, Prettau 4 Anterior showed greater color changes (ΔEab and ΔE00). None experimental group produced clinically perceptible ΔEab or ΔE00 values. Except for the GLAZE group brushed with conventional dentifrice, the translucency of the materials followed, in decreasing order, the inequality: e.max Press > e.max CAD > Prettau 4 Anterior. In general, the surface topography images revealed surface alterations, directly proportional to the increase of the years of thermomechanical aging, regardless the experimental group and dentifrice type, being more significant for the zirconia Prettau 4 Anterior. In general, the color, translucency and surface topography of pressable and CAD/CAM monolithic ceramics, extrinsically stained and glaze or glazed only, changed after toothbrushing with different dentifrices, followed by thermocycling.
