Apical periodontitis (PA) and smoking may be associated with metabolic syndrome, diabetes mellitus, and insulin resistance (RI). Melatonin (MEL) is a hormone with antioxidant and anti-inflammatory properties that also plays a role in bone remodeling. Previous studies have shown that MEL improves insulin sensitivity and insulin signaling in the skeletal muscle of rats with PA. In this context, the hypothesis was formulated that the metabolic and inflammatory alterations caused by PA would be more pronounced when associated with passive tobacco smoke inhalation, and that MEL administration could prevent or attenuate these changes. The aim of this study was to evaluate insulin resistance, insulin signaling, pro-inflammatory cytokines, inflammatory signaling pathways, and oxidative stress in skeletal muscle, as well as bone resorption, the intensity of the inflammatory infiltrate, and the immunostaining of osteocalcin (OCN) and tartrate-resistant acid phosphatase (TRAP) in the periapical region of adult rats with PA subjected to passive tobacco smoke inhalation and treated with MEL. A total of 128 Wistar rats, aged 60 days, were used and distributed into eight groups: control (CN); smoking rats (T); rats with PA (PA); smoking rats with PA (T+PA); control treated with MEL (MEL); smoking rats treated with MEL (T+MEL); rats with PA treated with MEL (PA+MEL); and smoking rats with PA treated with MEL (T+PA+MEL). Animals in the T groups were subjected to passive cigarette smoke inhalation for 50 days. On the 20th day of the experimental period, apical periodontitis was induced in the right maxillary and mandibular first and second molars of animals in the PA groups using a carbon-steel bur. Animals in the MEL groups were treated with oral melatonin (5 mg/kg) via gavage from the 20th to the 50th experimental day. At the end of the treatment, blood glucose, insulin levels, insulin resistance (via the HOMA-IR index), and plasma concentrations of the pro-inflammatory cytokines IL-6 and TNF-α were evaluated. In the gastrocnemius muscle (MG), the following were analyzed: insulin signaling (via the phosphorylation degree of pp185 (IRS-1/IRS-2); inflammatory signaling pathways (via the phosphorylation degree of JNK, NF-κB p65, NF-κB p50, and IKKα/β); and oxidative stress (assessed by superoxide dismutase (SOD), thiobarbituric acid-reactive substances (TBARS), protein carbonyls (PC), total antioxidant capacity via the FRAP method, reduced glutathione (GSH), catalase (CAT), and glutathione peroxidase (GPx). In the right hemimandibles, bone resorption, inflammatory infiltrate intensity, and OCN and TRAP immunostaining were evaluated. Data normality was assessed using the Shapiro–Wilk test. Based on the test results, parametric tests were employed, adopting a significance level of 5%. The results demonstrated that both PA and passive tobacco smoke exposure led to increased insulin levels and insulin resistance compared to the CN group. The combination of these two conditions in the T+PA group exacerbated glycemia, insulinemia, and insulin resistance compared to the T and PA groups analyzed separately; MEL reversed all these alterations. Following insulin stimulation, a decrease in pp185 phosphorylation was observed in the PA, T, and T+PA groups; this alteration was reversed by MEL only in the PA+MEL group. Regarding pro-inflammatory cytokines, no significant differences in IL-6 concentrations were observed. However, there was an increase in TNF-α concentrations in the PA, T, and T+PA groups, with a reduction in the MELtreated groups, except for the T+PA+MEL group. Regarding inflammatory signaling, an increase in JNK phosphorylation was observed in the PA and PA+MEL groups. NFκB p50 phosphorylation was higher in the PA group compared to the CN, MEL, PA+MEL, T+MEL, and T+PA+MEL groups; this alteration was reversed by MEL in the comparisons between PA and PA+MEL and between T+PA and T+PA+MEL. NF-κB p65 phosphorylation was higher in the PA group compared to the CN, MEL, and T+MEL groups. The T+PA group differed from the CN, T, MEL, T+MEL, and T+PA+MEL groups, and MEL reduced this parameter in the T+PA+MEL group compared to the T+PA group. No statistically significant changes were observed in IKKα/β phosphorylation. Regarding oxidative stress parameters, TBARS data showed that the T+PA and PA+MEL groups exhibited lower values than those observed in the CN and PA groups. In turn, the T+MEL and T+PA+MEL groups showed lower values than the CN, PA, and T groups. Treatment with MEL reduced TBARS values in the PA+MEL and T+MEL groups compared to the respective untreated groups, PA and T. No differences were observed among the groups regarding SOD, PC, and GPx. CAT activity was higher in the T+MEL group compared to the PA and MEL groups. GSH concentrations were higher in the PA group compared to the T+PA, MEL, and T+PA+MEL groups. The T group also exhibited higher GSH concentrations compared to the T+PA and T+PA+MEL groups. Regarding FRAP, the T+MEL group showed higher total antioxidant capacity compared to the T+PA group. Histometric analysis demonstrated increased bone loss in all groups with apical periodontitis (PA, T+PA, PA+MEL, and T+PA+MEL), with an exacerbation in the T+PA group and a reduction promoted by MEL only in the T+PA+MEL group. The intensity of the inflammatory infiltrate also increased in all groups with PA. Regarding bone markers, OCN immunostaining was lower in the T+PA group compared to the PA group. MEL increased this parameter in the T+MEL group compared to the T, T+PA, MEL, and PA+MEL groups. An increase in this immunostaining marker was also observed in the T+PA+MEL group compared to the T+PA group. TRAP immunostaining increased in the groups with PA not treated with MEL (PA, T+PA) compared to the other groups. Although the T+PA group showed lower TRAP immunostaining than the PA group, it differed from the other groups. MEL treatment reversed this alteration in the comparisons between PA and PA+MEL and between T+PA and T+PA+MEL. It is concluded that the association between tobacco exposure and apical periodontitis promoted more pronounced alterations in some of the evaluated parameters, especially regarding insulin resistance, insulin signaling, and bone resorption in the periapical region. MEL exerted a protective effect against most metabolic alterations resulting from PA and tobacco exposure and also demonstrated local effects, reducing bone resorption and modulating bone remodeling markers, which highlights its potential as a therapeutic agent.
