Avaliação das alterações funcionais e histológicas na isquemia renal, comparando-se duas técnicas de hipotermia: estudo experimental em suínos

Resumo

Cirurgias renais como nefrectomias parciais, transplante renal e cirurgia renovascular são situações clínicas que freqüentemente exigem a interrupção transitória do fluxo sanguíneo renal. A hipotermia renal é um dos métodos de preservação da função renal mais eficiente para se proteger as células renais das lesões de isquemia e reperfusão. O objetivo deste estudo foi comparar a técnica de hipotermia renal com gelo e a técnica de resfriamento renal retrógrado de solução salina à 4°C na via excretora. Foram utilizados 24 porcos machos entre 15 e 20 Kg, divididos em 4 grupos. Foram coletadas amostras de sangue e urina no dia anterior ao experimento e no 1° e 5° dia pósoperatório (PO), a fim de se medir creatinina e uréia, e se obter cultura de urina e débito urinário. Todos os animais foram submetidos à nefrectomia direita e, no rim esquerdo, foram aferidas as temperaturas do córtex e medula renal durante 60 minutos. Grupo 1(8): hipotermia retrógrada com solução salina à 4°C por meio de cateter ureteral duplo lúmen com obstrução do pedículo renal; grupo 2 (8): hipotermia com gelo e isquemia renal; grupo 3 (4): apenas isquemia quente e grupo 4 (8): controle. Os animais foram sacrificados no 5° dia e os espécimes cirúrgicos submetidos à análise histológica de microscopia óptica (MO). Todas as culturas de urina foram negativas. No grupo 1, em média, as temperaturas mínimas das camadas medular e cortical foram respectivamente 24,9 e 25,4°C e no grupo 2, foram 6,38 e 5°C. Para as variáveis uréia e creatinina, foi aplicada análise de variância e para a histologia, o teste exato de Fisher. Tanto na comparação entre os grupos 1 e 2, quanto na comparação isolada de cada um deles com o grupo controle, não se encontraram diferenças funcionais e histológicas estatisticamente significativas...
Renal surgeries such as partial nephrectomy, renal transplant and renovascular surgeries are clinical situations which often require the transitory interruption of renal blood flow. The renal hypothermia is one of the methods of renal function preservation which is the most efficient one in order to protect renal cells from lesions of ischemia and reperfusion. In 1975, Ward suggested that 15ºC would be the ideal temperature to cause regional renal hypothermia.(1) Albeit there are several methods of renal hypothermia, it is traditionally obtained by renal cooling involving the kidney with ice slush after the obstruction of the renal artery.(2-4) Nowadays, laparoscopic renal surgeries have developed a lot, which turned it to be an alternative to partial nephrectomy (LPN).(5) The technique of ice slush was reproduced to the laparoscopy, but presents technical difficulties in order to put the bag and to the introduction of the ice, which makes the method difficult to handle. Perfusion of cold solution in the renal artery is not widely used due to the risks of lesions in renal vessels.(6,7) Landman et al proposed a model of retrograde endoscopic intracavitary cold saline perfusion.(8) It is an easy applicable technique and it allows getting a resection area clear free from ice and it avoids the cooling of anatomic structures which involve the surgical area. However, this technique brings doubts about its capacity to produce the ideal temperature in the proximal convoluted tubule, once it cools the renal medulla better than the cortex.(9,10) Manuscript 2 73 This study intends to compare the Landman’s technique with the traditional method of cooling with ice slush and to evaluate the functional alterations of the kidney and the histological alterations of cell brush borders of the proximal convoluted tubules of pigs kidneys.
