Resumo
O objetivo deste estudo prospectivo foi avaliar cefalometricamente as alterações esqueléticas e dento-alveolares naturais e induzidas pelo uso do Ativador Elástico Aberto de Klammt no tratamento da má oclusão de Classe II divisão 1. A amostra foi constituída por dois grupos pareados segundo gênero e idade: grupo experimental com 17 indivíduos (10 meninas e 7 meninos) que receberam tratamento com o aparelho de Klammt pelo período de 12 meses e grupo controle com 17 indivíduos não tratados (10 meninas e 7 meninos) observados durante um período de tempo similar. A média de idade inicial foi de 8,5 anos para ambos os grupos. Telerradiografias laterais foram obtidas no início e no final dos períodos de tratamento e observação. O grupo controle foi obtido a partir dos arquivos do Departamento de Clínica Infantil - Ortodontia, da Faculdade de Odontologia de Araraquara, Universidade Estadual Paulista - UNESP. As alterações observadas foram comparadas por meio do teste t de Student, a análise de variâncias foi realizada pelo teste de Levene e a relação entre altura do acrílico oclusal e alterações esqueléticas e dento-alveolares foi avaliada pelo coeficiente de correlação de Pearson. Os resultados demonstraram que o aparelho promoveu restrição dento-alveolar da maxila; aumento do comprimento mandibular total, comprimento do corpo mandibular e altura do ramo sem alterar o ângulo goníaco; aumento das alturas faciais anterior e posterior sem alterar o padrão facial de crescimento; melhora na relação maxilo-mandibular; verticalização dos incisivos superiores; manutenção da inclinação dos incisivos inferiores; redução do overjet e overbite; correção da relação molar de Classe II. The objective of this prospective work was to evaluate cephalometrically the natural skeletal and dento-alveolar changes and those induced by use of the Klammt's Open Elastic Activator for class II division 1 malocclusion treatment. The sample consisted of two groups paired according to gender and age: treated group with 17 individuals (10 females and 7 males) that received treatment with the Klammt's appliance for period of 12 months and control group of 17 untreated subjects (10 females and 7 males) observed during a similar period of time. The initial mean age was 8.5 years old for the both groups. Lateral cephalometric radiographs were obtained at the beginning and end periods of treatment and observation. The control group was obtained from the files of the Department of Children's Clinic - Orthodontics at Araraquara Dental School, University of State of São Paulo - UNESP. The observed changes were compared by the student's t-test, the analysis of variance was carried out by Levene test and the relationship among the oclusal acrylic height and the skeletal and dento-alveolar changes was analyzed by Pearson's correlation coefficient. The results demonstrated that the Klammt's Open Elastic Activator produced dento-alveolar restriction of maxilla; increase in total mandibular length, body mandibular length, ramus height and no change gonial angle; increase in anterior and posterior face height and no change growth facial pattern; improvement in the maxillomandibular relationship; palatal tipping of the upper incisors; maintenance of the tipping of the lower incisors; reduction in the overjet and overbite, and correction of Class II molar relationship.
